I usually do not recommend surgery unless the vision is worse than 20/50. If this is the case, then discuss the risks and benefits with your surgeon. Usually macular holes don't get better on their own past a certain stage and will cause permanent vision decrease if they stay in place for too long. Discuss all these points with your surgeon.

There are way too many factors that need to go into such a decision and discussion. I'm assuming you mean a macular hole rather than tear. It depends how bad the vision is and what other findings there are in your eye that may increase or decrease the risks. You need to discuss these at lenght with your retina specialist. If you're still not sure, then get a second and maybe even a third opinion until you feel your concerns have been fully addressed. You could find a list of eye MD specialists on www.aao.org.
