ovulating only after the trigger

Hi
I'm 33 years old and having trouble getting pregnant. I'm likely to start down the IVF route soon but I'd like to know the reason for something and if it will have any impact on the IVF (esp what drug protocol would likely be most effective). It seems that I only ovulate when I've taken the trigger (HCG) shot. I'm monitoring my cycles with the clearblue fertility monitor. My cycle usually goes: period, then rising oestrogen levels...which stay high but I don't ovulate. Then eventually they lower and my next period comes. It seems like I am not producing the hormone that matures my eggs and releases them, but what does that indicate? I've been diagnosed hypothyroid (on medication for 6 years) and I have PCOS as well. Thanks for your help!

Thanks Dr.Brown! What would you suggest in terms of treatment for this? I'm already taking metformin (1500mg/day). I'm particularly interested in knowing what this might mean for which IVF protocol might work best for me. I have an appointment with an IVF doctor 7th Jan but would like to go to the appointment informed! Thanks a lot.

Hi!
If you were my patient, I would require a bit more information to give you a clear accurate answer, but maybe this will help.  In some cases of PCOS, women will have many follicles begin to mature, but none will become dominant over the others--as should happen in a normal cycle.  There may be lots of immature follicles, but none large enough to rupture and fertilize.
This accounts for an elevated estrogen level but a low progesterone level in PCOS cycles.  My theory can be documented by watching the follicles with transvaginal ultrasound.
