semen

i am male aged 42. i have 2 kids already and after a gap of 6 years, we wanted one more pregnancy and i gave up the condoms but we failed after the continuous try of 5 months. then i got some medical advice, Doctor advised me semen analysis and it was shocking for me that my sperm count was 6.2 million per ml only and motility  was almost zero.my doctor suggested medicines clomiphene citerate 50 mg one tablet daily for 30 days,azithromycine capsule twice daily for initial 7 days, and one capsule daily of prime rose oil for 30 days.he also advised me more butter and milk in diet and more healthy food.he permitted me for sex as my previous routine and to stop sex before semen analysis again.
i am worried about my sperm count and motility, please help and guide me.

I have a question about my sperm count which was also low. My count was around 2mill/ mL, my Volume was normal, Motility and Morph were extremely low.  My brothers have both had testicular problems, one with torsion and the other a confirmed vericocele. I have had all the tests and all Neg (-). The only thing that I can think of for my low count is that I was on Acutane as a teen and I know there are warnings for child defects in women taking acutane but didn't know if there were also male fertility side effects?
We have been trying for 3 years and IVF is something we are looking into possibly this year. Thanks for your help.

I agree with the plan to repeat your sperm count. I would recommend seeing a board certified reproductive endocrinologist in an infertility program where they can do the semenanalysis in house. If your second count is again low, then you will probably requre IVF, and also should have an evaluation by a urologist to ascertain if there is a problem with your testicles causing the low count. Usually this is not the case, but needs to be checked. Clomid occasionally helps, but most of the time the best treatment when a sperm count is very low is usually IVF. Finally, commercial labs can sometimes be off. When collecting the count again, you should abstain 2-3 days, and have the sample at the place where it is being analyzed within an hour of collection.
