low sperm motility

hey..my husband got his semen analysis done today.the results are--
SPERM CONCENTRATION--  42 MILLON

NORMAL SPERM MORPHOLOGY-- 87%

SPERM MOTILITY-- 13%

his sperm motility is just 13% which is very low. He is 36 year old and we are trying for a baby...i am really disturbed seeing the results. Please guide what we should do.

Hi roger,

i done my semen analyasis and found low sperm count(7 lack) and motility iis only 10%.
Out of these only 5 % are repid motile and 5 % slow motie while 80% are immotile.
Morphological only 50% are noraml

My dr suggest me fertisure M / Fertyl M , Tab Doxt OD , Tab Exebid BD, Lycored OD for 3 days.

I doing Gym regulary and taking Amino supplimets.

In how much time my sperm will be normal

i had done my seman , it shows result  50 m/ml and 40% mobility. 9-10% pulls . is it ok for me  to get pregnet my wife.

yup..i know our situation is unusual...but i guess you are right...cos whatever 7-8 times we tried, initially for 5-10 mins, he will be erect but not hard enough...n then completely loses it...

jus a doubt...i hav heard abt Viagra n stuffs...but jus wanted to confirm is it ok to use especially since we want to hav a child...i mean i hope it does not affect the male fertility in anyway?? n second doubt is u asked us to consult sexologist...is it same like Urologist...cos we done normally find a sexologist in the hospital....

anyway, thanks for all your time n effort to help me out...it really means a lot n i jus pray things work out soon...

I'm sorry to say this. But first you guys have to make the marriage work.The child will come automatically. Please enjoy sex for what it is. If you have it for the sake of a child, it becomes tiresome and very soon the marriage will on the rocks.

yes I find your situation unusual. In the indian context, I find it even more astonishing as to how you managed family, friends & strangers (for 4 years) who constantly badger you with questions on kids !!!

You guys have to see a marriage counsellor. Better yet, consult a sexologist. I think your husband has performance anxiety. Over here at my clinic, my consultant usually asks the guy to start off with viagra 50 mg. In a months time the guy learns how to do it and he comes of the drug. This has worked beautifully in 90% of the cases (where the problem is in the mind). you can try this out under the supervision of a qualified medical professional.

hey..thanks for ur reply...ar u guided, we are going to see a urologist tomorrow....i hav stopped his coffee also...he does wear boxers only n of course never places the laptop near genital ares....

one thing i would like to say is that though we hav been mmarried for 4 years, but till now we have never had sex..it might sound quite strange but yes...its true...initially i was away for my studies...n he travels a lot cos of his work...so we hav never had sex. but recently we started trying cos both of us want to hav a baby now...we tried 7-8 times bu cudnt succeed as he wasnt hard enough...he cud get erect initially but when it would ocme for penetration, he would go completely soft... we tried 7-8 times but then thought to give a gap. i think it might be ED or may b it happens to guys when it is their first time...dont know...

basically to be honest am very disturbed cos everything is in a mess...he doesnt drink, doesnt smoke...he doesnt hav any medical history..no bad habits...a healthy lifestyle n a healthy routine....yet we are facing all sort of problems...earlier i was disturbed cos he cannot get hard enogh to penetrate...and now to add on to my troubles is his low sperm motility...

i am 26 years old and he is 36...we badly want to hav a child now n i jus dont know what to do as this is something i cannot share with anyone....

Hi Neha,

Your report is as per WHO 1992 standards even though WHO has revised the same in 2010. Sperm count and morphology are excellent even by the old 1992 standards. All other parameters are okay. I suggest the following steps

(1) Immediately consult an urologist to rule out a varicocele. Insist on a doppler ultrasound of your DH's testes as most doctors limit themselves to a physical examination of the testes only.

(2) Immediately change your lifestyle as suggested above - (a) No Coffee / alcohol (b) No cellphones in trouser pockets (c) placement of laptops / tablets as far away from the genital area as possible (d) Wear Boxers only (e) Add a lot of greens to your diet. (f) Drink lots of water (g) mild to moderate intensity exercise (h) Have regular sex. say every 2 to 3 days. (i) Ask your DH to ice his family jewels every night.

(3) Vitamin cocktail - Ask him to start taking these vitamins (1) Carnitine - Brand Fertisure M / Carnitor  - 1 tab thrice daily (2) Lycopene - Brand Lycored - 2 Cap thrice daily (3) Vitamin C 500mg - Brand - Selin 2 tabs in the morning

(4) OPTIONAL STEP - Evaluate your DH's sexual performance. Ask him whether he observes any changes from say a year ago in (a) the quality of his erections (b) morning wood (c) recovery time after having sex (d) the actual sexual act itself. Check whether there are any changes.

If your husband believes that there are indeed some changes, get the following  checked to rule out any harmonal imbalance (i) Total testosterone (ii) Free testosterone (iii) LH (iv) FSH (v) Thyroid panel (vi) prolactin. Harmonal imbalances usually manifest in sperm motility first before becoming apparent in other factors.

Start him on the vitamin cocktail and get SA done after a month. Sperm get the ability to swim in the last stage of production only so no need to wait for 3 months. Get him tested after 1 month.

And finally cut down stress. Stress is the most damaging when it comes to fertility.

If 13% is the total motility figure, then it is bad. However if it refers to the rapid motility figure, then your husband's numbers are excellent. Please post the entire report including motility grading for us to help you. Were there any pus cells in the semen? Was liquefaction complete? Is there any time mentioned along with the motility figure.

Even if total motility is low, I personally dont think there is anything to worry about as the count & morphology are awesome. Here are a few things your DH can do (1) Switch to boxer briefs (2) no cell phones in trouser pockets (3) Regular orb-icing (though temperature is not the issue here as far as I can tell) (4) Sex every 2 to 3 days (5) Drink lots of water (6) start him on multivitamins - highly recommended (a) Carnitine (b) Lycopene (c) vitamin C
