Smoking increases your risk of many diseases unrelated to whether you have your gall bladder or not.  Birth control pills should not increase your risk for heart disease just because you don't have a gall bladder anymore.  If you have high blood pressure and are taking estrogen based birth control pills, you are at a greater risk for heart attack or stroke...again unrelated to having a gall bladder or not having a gall bladder.
