Abdominal pain. Please help me.

Hi all,

I've been having some weird abdominal pain over the past 2.5 years. The severity comes and goes in waves, but I believe it is directly tied to exercise level (will discuss a bit later).

Age: 33
Sex: M
Height: 5'11
Weight: 200

The pain is on the right side, and the center is about 2 inches to the right of my belly button. I've attached an image. Red circle = primary pain area, and yellow circles = also have some pain/discomfort there. (https://imgur.com/a/1mSCuMi) The sensations are as following: sometimes pinching, other times it feels like a dull cramp, and sometimes a pulsating type of pain. It's definitely enough to make me focus on it all day. I would say it's about 2.5/10. Sometimes it radiates upwards and outwards, almost like it's going around to my back.  I also have a sore right testicle (pain is of varying severity, sometimes only painful to touch, sometimes a dull throbbing feeling), and a tightness/achy feeling down my right hamstring.

The pain bothers me when I'm exercising heavily, but is also made worse by going from laying down to sitting up, and anything that requires me to bring my knee up high towards my chest.

This leads to a general feeling of being sick/malaise. It isn't something like a muscle strain that just causes an ache but feeling fine otherwise, but a much more broad feeling of illness in addition to the pain. The symptoms are worst after exercise: especially strenuous, like sprinting, high resistance elliptical, etc. I've only felt close to "normal" for a couple of weeks over the past 2 years. The first time was when I injured my back and could not exercise for 3 weeks. The next was when I injured my leg. The day I resumed exercise after the leg injury, I started feeling sick and getting the pain in my abdomen.

I've had a CT scan which revealed slightly enlarged lymph nodes in the area, but no hernia (oddly enough, I do have a small hernia on the left side, but no pain there). I've had a colonoscopy, which was clean.

I have no idea what to do, but every day feels like a challenge to get through.
