I have a variety of symptoms and still no diagnosis, anyone have any ideas?

Hi, I'm a 26 year old male with a history of thoracic back pain, shortness of breath and palpations going back 2-3 years. I used to think this was all anxiety due to chronic back pain until the last 4 months. Ive lost my job it's gotten so bad. When it first hit the fan I was constipated for a week. Then I had tachycardia so bad I couldn't even stand up and it's still a gamble to do. I have this intense pain and pressure in my chest right in the center near my sternum, I feel pain in my ribs and back and it feels like there's a rubber band around my diaphragm making it hard to breathe. When I drink or eat I feel the material go down and stop right around my sternum area and the pain in my chest increases, seconds later I'll feel and hear it slide down the rest of the way. My pain is typically worse after eating and when moving around. I do have some acid reflux that is happening. Seconds after standing up I'll feel my heartbeating really hard and it feels like my whole head is a heart beating, it is usually accompanied by tachycardia and lightheadedness. Maybe a minute after standing up that rubber band feeling around my chest, ribs, abdomen and back increases and it's incredibly hard to take in a breath, like the muscles that make the chest rise and fall are super tight. It also constantly feels like I have to burp and burping tends to temporarily dull the pain. At times my heart skips beats(pcv or PAC) and it makes my chest hurt that much more. Those are my chief complaints but aside from that I have little twitches that happen all over my body, pressure in my head that causes dizziness, ringing in my ears with temporary hearing loss in one ear, pins and needles in my legs into my feet and when it's real bad I can feel my body shaking it's not noticable unless your looking for it but it's easily felt.

I've seen several primary doctors, been to the ER too many times and have been told over and over that I'm just anxious by er doctors. The issue there is these symptoms have lasted for 4 months straight with no break to varying degrees of severity. It's like living in hell. I just got off using a 30 day holter monitor and have appointments with gastroenterologist and neurologist coming up. Went to the Pulmonologist and he said I have asthma of the small airways which of course he said he cannot test for. Had an angiogram of my chest and they said it's all clear, H pylori test was clear and so on and so forth.. I'm on beta blockers, inhalers, omeprozol, baclofen and docusate sodium BC occasionally I get constipated. Obviously my anxiety about all this makes it worse but I'm racking my brain about what the deal could be. Does anyone have any thoughts on what could be wrong here. It would help to know questions I could ask my doctor to maybe speed this along. Have any of you experienced any of this? Any potential answers would be awesome. Thank you
