anemia, iron infusions, blood transfusions, paraesophageal hiatal hernia

Frustrated.  Diagnosed with paraesophageal hiatal hernia ~2or  yrs ago.  Anemic, requiring iron infusions every week and have had 5 blood transfusions.  Stool cards fo r occult blood have been half +, half -.I am obese...200 lbs.  Titrating down off prednisone for question of polymyalgia rheumatica.  Dr. refuses to operate due to obesity and prednisone.  Any suggestions.  Am trying to lose weight.

Hi there!

Well, without knowing the relevant clinical details or a detailed clinical evaluation it would be difficult to comment specifically on the situation. I would suggest considering an initial evaluation by an internist or a haematologist to classify the type of anemia. If bleeding/ hemorrhage is suspected as a cause, further investigation to look at the cause of bleeding can be planned. For GI bleeding upper and lower GI endoscopies may be suggested as primary investigations. I would suggest discussing the situation and the suggested management plan in detail with your treating doctor.
