stomach pain

for more than two years i have stomach pain     it use to start at lower left abdomen   now it starts on left but midway across from navel
when it is real bad bulge appears at my navel
then i have trouble passing gas
is it IBS  or hernia problem or adhesion or celiac or gluten or disseminated histo or tb or hiv
i have stable legion on my right lower lobe and sores on my tongue i have had laproscopic tubal ligation and breast cancer  i am 61 yr female clinical lab worker  thank you
