surgery operation hiatus hernia

Dear NHS Staff,

thank you so much for entering in my post

I am a guy of 28 years, I've been suffering for 7 years from acid reflux with hiatus hernia of 1,5 cm of diameter, diagnosed by endoscopy made on date 27/06/2009
my symptoms are:

1)pain in the central part of my stomach with presence of a sort of little ball which is visible even to naked eye
2)feeling of foreign body in the throat
3)feeling of burning to my oesophagus
4)rarely small spills from the mouth of gastric

My GP doctor gave me a therapy of Lansoprazole caps (ec grans), 15mg.

I asked my doctor about a surgery operation, and she told me that here in England they don't do it any more this kind of operation, and she added they do it just when the hernia is very big

When I write down a post in medicine forums about this illness every doctor says that the methods to do this kind of operation are several, and it's possible to do it when:
the patient is young and he doesn't want to take medicines for all his life,
the patient has got regurgitation and he can't control it with medicaments,
or when the patient decides to do the operation for his choice

So why when I ask about it in physical environment like hospitals or doctors they say me that is not possible, and in medical forums they say me totally the opposite

I really want to do this operation
My life is very limited with this problem: I cant even go for a drink with friends as how painful it is, sometimes I can't drink even cold water  

Most of the time I have to be sit at my work and sometimes I fall asleep on my chair because I can't even drink a coffee, coffee is like poison for my stomach

Sometimes I have all my throat is irritated and I have problem to talk to people

Why I have to suffer all my from this illness if a solution is possible?

Any information concerning this will be very appreciated

There are two types of hiatal hernias. One is a sliding hiatal hernia (which is very common in about 15% of the population and invasive surgery is not required) the other is called Para-esophageal hiatal hernia and it is very rare in adults and more commonly found in infants as a birth defect.

I have two questions for you:
Which are you diagnosed with?
What has been determined as the underlying cause?

Under normal circumstances GERD can be very easily controlled with medication. Lansoprazole is a very effective drug so perhaps you need to discuss with your doctor about increasing the dosage or even changing medications. Famotidine works very well for me.

I understand your frustration about how this hinders your lifestyle and I can tell you that this can all change when being properly medicated. Surgery should not have to be the option you are looking for. Your doctor just needs to know so he/she can do more to help you live a comfortable lifestyle. You should also keep in mind that smoking and alcohol are two major causes of the sphincter relaxing causing the GERD. This can become dangerous in time as the stomach acid being produced can cause deterioration. I don't think anyone likes having to take medication for the rest of their life but under this circumstance it is your best option. Talk to your Doctor stressing how much pain you are dealing with. Yes I know about nissen fundoplication.  If you are considering this surgery, I would recommend researching it in depth.  It is a surgery that can have A LOT of nasty side effects and it won't necessarily cure you.

But I don't know about it here in England, if it's possible to do it or not

Dear Randy,

thank you so much for getting back to me

1) I've been diagnosed with  sliding hiatal hernia of 1,5cm of diameter
2) The causes are not mentioned on the medical certificate about my endoscopy made on date 27/06/2009, but I know them: I used to study opera, and a bad use of my diaphragm caused this hernia
I want to do this surgery operation I've been suffering from this illness for 7 years and I can't take it any more now
I'm going to talk again with my GP Doctor about it and I hope I will solve this problem as soon as possible

I think what you are referring to is a nissen fundoplication.  Is this correct?

Basically I need to reduce this valve between the stomach and the oesophagus from my last  endoscopy made the 27/06/2009 it was 1,5cm of diameter I don't know now, for me it doesn't matter how,
yes I know already all this kind of methods you wrote down

Which surgery are you referring to?  Are you referring to a surgery for GERD (nissen fundoplication) or a surgery for the hernia?

yes I know what you mean about GEND diet

Yes I tried to avoid all lactose, spicy foods, fatty foods, caffeine, etc... but I haven't tried any other medications besides Lansopranzole
But I know also that this kind of therapy by medications needs just to maintain but it doesn't solve the problem, so I should take this medicines for all my life and  don't want to do that

That's why I want do to this surgery operation, this problem doesn't allow me a normal life

Welcome to the gastroenterology community!  Almost all of your problems could be caused by the GERD.  Are you on a GERD diet?  Do you avoid all lactose, spicy foods, fatty foods, caffeine, etc.?  Have you tried any other medications besides lansoprazole?
