Zithromax is an antibiotic. Has he told you there is an infection - kidneys perhaps? and six months is a long time. I would make the break now and get a new physician, since this fellow is retiring soon anyway. Ask some of your friends for a recommendation, or choose an Internist who is able to address all of these issues. I suspect your doctor is a general practitioner who has not kept up with current treatments. Antibiotics are not for back pain without a definitive diagnosis, and should you develop a severe infection, you could possibly develop an immunity to their healing properties if taken for six months. Make some inquiries or calls today. You may have kidney stones, an kidney infection (which shouldn't take six months to cure) or it could be good old arthritis, even though you are young. So I looked up some things regarding celiac and B12 and these are some things to note: B12 deficiency and low thyroid causes fatigue.
Regarding B12, the deficiency can come from lack of absorption or anemia. Also, Atrophic gastritis affects 30% of people 50 and over. I don't know your age but apparently this is a thinning of the stomach lining so you don't absorb B12. Celiac disease can also cause B12 deficiency.
But since you have celiac disease it seems to fit in that celiac and low thyroid are seen together, and also celiac causes B12 deficiency. It can also cause mouth sores (canker), and affects hormones so that may be why you have the back acne. It can cause late puberty and problems with menstration and so forth. Gluten can trigger it in some people. I think overall it can lower your immune system too. So this explains the yeast infection. So, I think you have to keep the celiac under control and be supplemented with B12 so get tested regularly and same goes for the hypothyroidism. Either get shots of B12 or take sublingual (dissolves under the tongue) B12. The best absorbing type is Methylcobalmin. But I would ask your doctor how much. If you don't have a doctor you can take sublingual once a week, 5,000mcg. Then get tested in 3 months. If you are very low it is best to get a shot to start with.

Low thyroid and low B12 will give you fatigue. If you are borderline on the thyroid you can do better. Are you on Synthroid? You should be down around 2.5 or less if possible for the TSH. It is important to find the cause of the B12 deficiency. I hope you are on low dose Zithromax for such an extended period. Are you sure you had thrush because sometimes a coated tougue can look like yeast? Were you on antibiotics at the time because that can cause it. Also, a lowered immune system can cause it too. Let me think about the B12 deficiency and your symptoms and see if I can think of anything else.
