Abnormal bleeding for over a year now...

I am a 34 y/o mother of 1. I’ve been on BCP for 12 ½ years. In Aug 2007 I started taking Kariva 28 day and stayed on it until Aug 2009. A few months before this, I began having issues with my cycle. I wasn’t getting my period, I got my period early plus the full week I was supposed to, and then I was spotting several days a month. I then found out I had Hashimoto's Thyroiditis and started treatment hoping that would help. I didn't. I went on Portia 28 for 5 months, Junel 1.5 30 tab for 3 months and now I’m on Junel 1/20. Next week will be the end of the 2nd month on that for me.
   I’ve had light to moderate spotting the week before my period then a full period the week of placebo pills. I’ve had some days of horrible cramping and bleeding through several tampons AND pads each hour for a couple hours. Last month when I started the new pill, I had spotting the week before my period and then I had my actual period for only 2 ½ days the next week. I thought this was the fix I needed.I was wrong.
   This month, my second week of pills I had some light spotting where I didn’t need anything followed by a day or two of needing a tampon, followed by a day now needing a panty liner. I’m due for my period next week and am not sure what to expect.
   I’ve had an internal and external ultrasound done. The result was that there were normal, but that the lining was thickened. My pap’s always come back normal. I have my PCP who is a RNP do all my GYN care, but have just made an appointment with a specialist for this week. I’m wondering what I can expect, or what I should be asking. My paternal grandmother died from a reproductive cancer when I was 2 (no one knows for sure which kind!) and my sister has had several abnormal pap’s where she has had to undergo a procedure to remove the cells. My mother and her mother both have/had uterine prolapse.I just don’t know if I should be concerned about all of this. Or if this is just a part of life I have to deal with. .

Thank you for your response Dr. Brown. I saw a specialist yesterday who informed me that from my last ultrasound report (9/09) my endometrial lining was 2 mm. It is listed as a heterogeneous uterus with no discrete lesions or masses. She is sending me for another pelvic ultrasound with TV as well. She also said that if the lining is still this thin, that she will want to change me to a pill with more estrogen. If it's normal, I guess we go from there.
I'm now in my second week of bleeding this cycle and am due to have my pill-period next week. I think I am not going to refill my pills next month and just wait for the outcome of the ultrasound. I did stop the pill for 3 months last year, but didn't have any luck with it at that time.
My only other question is what does heterogeneous uterus mean?

Hi!
If the lining is too thin, you need more estrogen.  If it is too thick, you need more progesterone.  If you were my patient, I would try giving you 14 days of progesterone alone and then re=start pills.  If this didn't work, I would consider a biopsy of the lining to rule out a polyp(s).  One other possibility that would probably work is to stop pills altogether and use condoms for 3-4 months.  That might let your body have a chance to re-set your cycle.
