abnormal menstrual cycles

I have a gyno. appt.scheduled for the end of the month (that long to get in to the doctor as a new patient) and in the meantime I have been to my GP who did blood cell count and hormone  tests. All came back normal. He also scheduled an ultrasound 3 years ago diagnosed with thickened uterine wall..no recommendations.  Saught medical attention at this time due to change in my cycle (no period, spotting for 3 months and then a period that lasted 3 weeks).  Put on a 4 day dose of progestin, periods resumed as normal.  My history over the past six months :Oct15 - 20th (heavier than normal - no pms)... I have never been bothered with PMS,  The only sign I usually got was very sore breasts a week before my period (the pain subsided as soon as my period arrived). No pain in breasts.

Nov 10 - 4- lighter and no breast pain --streaked blood mixed iwth slimmy discharge
Dec 6--10-light
Jan 26- Feb 22nd - small blood clots in wk 3. large blood clot then period stopped
Feb 26-Mar 4 - small clots during third wk, a few larger clots with increased pushing during  urination or defecation. Periods stopped again immediately after lost of huge blood clot.
Mar 4th - 8th- clear discharge (Some occasional minor vaginal itching )
Mar6th- wiped after urination- pink ting on paper
Mar 13 same as above
Mar 14 (today)- some cramping/sharp pain in lover pelvic region (like on top of pelvic in p ubic hair region), some burning- light period
I had a endometrial biopsy (actually and attempted one several  years ago) and really do not want to have one again..it was extremely excruciatingly painful (never had children, uterus not straight and had difficulty inserting tube into vagina) I  passed out. I am afraid this is going to be suggested again. Is there an alternative methods to obtain a biopsy without going through this pain (I rather be knocked out than to go through this again)?

What is your best advice about my whole situation.? 43 years old

I went for my ultrasound today..no abnormalities (lining looks good no fibroids, polyps. They did see a cyst on rt. ovary. May recommend follow-up. So why do I continue to bleed. Blood work showed no changes in hormone levels.

Hi!
If you were my patient, I would suggest that we go straight to the OR for a D&C/hysteroscopy.  That can be done under general anesthesia, regional anesthesia, or heavy IV sedation.  You will need some tissue for biopsy, but no need to be tortured to obtain it.  That's why we have anesthesia!
The other thing to consider, is what to do to prevent the thickening from coming back afterwards--for many people, taking progesterone for 14 days every three months works very nicely--and then follow the lining size with ultrasound.

Make sure you tell your gyn doctor that you have cervical stenosis ( closed cervix ) and that you passed out at the last attempt.  If they want to try the biopsy anyway--tell them you have an appointment to get to and will come back next week--then find another doctor!

Good luck!
