Cumnilingus with menstrual blood

Hi Dr,

I'm panicking a lot. I'm a man living in thailand.

3 days ago, I performed oral on a CWS during around 1 min while she was a bit menstruated. I was very drunk so I don't remember if she's menstruated a lot or no.

1) can you please assess my risk considering both little/very menstruated
2) I'm very confused about HIV transmission through cunnilingus. Some experts say low risk, some say no risk.
3) also,any experts say that when a women is menstruated, the risk is real, not only theoretical. What's your view ?
4) I'm planning to do an PCR RNA 10 days after exposure. What specs of the test should I ask to ensure I do the right one? Number of copies ?
5) why so many experts do not recommend PCR RNA?
6) if the test is negative, can I move on?
7) what is your view of my situation? Do you consider my event to be low risk or no risk?

Dr, I really don't feel well after this event, hence your help is really appreciated

Thank you for your post.  Here are your answers as follows:
1) can you please assess my risk considering both little/very menstruated
THE RISK OF PERFORMING CUNNINGLINGUS ON A HIV POSITIVE WOMAN IS CONSIDERED TO BE ZERO OR VERY CLOSE TO ZERO.  WITH SIGNIFICANT AMOUNT OF INFECTED MENSTRUATED BLOOD AND POOR ORAL HYGIENE, THE RISK WOULD STILL BE CONSIDERED TO BE LOWER THAN PERFORMING ORAL SEX ON A POSITIVE MAN WITH EJACULATION IN THE MOUTH, AND THIS IS ESTIMATED AT LESS THAN 0.04%.  THEREFORE IF ANY RISK, IT WOULD ALWAYS BE LESS THAN THAT.
2) I'm very confused about HIV transmission through cunnilingus. Some experts say low risk, some say no risk. AS MENTIONED ABOVE, MOST STUDIES SHOW THAT THE RISK IS ZERO.
3) also,any experts say that when a women is menstruated, the risk is real, not only theoretical. What's your view ? THE RISK IS LESS THAN IN THE CASE OF PERFORMING FELLATION WITH EJACULATION IN THE MOUTH, AND THE RISK OF THIS IS BETWEEN 0 AND 0.04%.
4) I'm planning to do an PCR RNA 10 days after exposure. What specs of the test should I ask to ensure I do the right one? Number of copies ?
WE USE THE QUALITATIVE MULTIPLEX PCR RNA TEST, WHICH DETECTS 40 COPIES/ML AND ABOVE.
5) why so many experts do not recommend PCR RNA? IT IS NOT A DIAGNOSTIC TEST AND IT IS NOT CONSIDERED CONCLUSIVE AS IT SHOULD ALWAYS BE FOLLOWED BY A DUO TEST AFTER 4 WEEKS, EVEN THOUGH IT HAS A RELIABILITY OF 99% AT 10 DAYS.
6) if the test is negative, can I move on? YES, BEARING IN MIND THE VERY LOW RISK OR NO RISK OF YOUR SEXUAL ENCOUNTER.  BESIDES YOU DO NOT KNOW THE STATUS OF THE LADY.  IT IS FAR MUCH MORE LIKELY THAT SHE IS NEGATIVE ANYWAY.
7) what is your view of my situation? Do you consider my event to be low risk or no risk?
I WOULD SAY NO RISK.

1) how you defined poor oral hygiene? I'm an heavy smokers and I brush my theeth aggressively, is it consider as poor oral hygiene?

2) considering my event, would you recommend an HIV test?

3) have you ever heard of a case where HIS has been transmitted through cunnilingus ?

4) do you think I'm panicking for nothing ?
