Follow-up again...sorry :(

Hi

First, I would like to sincerely apology for writing a new post. It’s been 20 days I’m not sleeping because of my unprotected sex with a Thai prostitute. Here’s my last post:

• I heard that the PCR RNA depends a lot on the skills of the lab performing the test. I’m worry that here in Thailand, they don’t use state of the art technician. Is it true that the PCR RNA method varies? Do you think there’s a possibility that the 99% accuracy after 10 days you mentionned cannot be applied here? The PCR RNA is my only source of comfort now and I’m even sure I can count on it since it seems that it is a hard test to read and I’m afraid the standard here are different from the west?

• The hospital told me that the PCR RNA I did doesn’t test for HIV2. Should I do a test for this?

• I want to do a DUO on day 21. If negative, is there a lot of chance of positive on day 28?

• I also want to DUO on day 26 as well. If negative, is there a lot of chance of positive on day 28?

• I would like to understand why DUO test in UK and US is considered conclusive given the 99.89% reliability that many experts seem to agree with?

• When I ask for a DUO test, should I ask for a specific method or only ask for a 4th generation?

• What’s your opinion on my situation?

Thank you for your post.  I will answer each of your questions here below:

I CANNOT REALLY COMMENT ON THE QUALITY OF TESTS IN THAILAND, BUT I AM SURE THAT THE TESTS USED THERE ARE SIMILAR TO THE ONES WE USE HERE IN THE UK.  IF WE COMBINE A NEGATIVE PCR RNA TEST AT 10 DAYS WITH A SEXUAL ENCOUNTER LIKE YOURS INVOLVING A RELATIVELY LOW RISK, WE CAN CONSIDER THIS CONCLUSIVE.

• The hospital told me that the PCR RNA I did doesn’t test for HIV2. Should I do a test for this? HIV-2 IS EXTREMELY RARE, MAINLY ONLY FOUND IN WEST AFRICA.  THEREFORE I WOULD NOT WORRY ABOUT IT.

• I want to do a DUO on day 21. If negative, is there a lot of chance of positive on day 28? NO, THERE IS NOT, ESPECIALLY AFTER HAVING HAD A NEGATIVE PCR RNA TEST AT 10 DAYS.  THE P24 ANTIGEN IS ALREADY DETECTABLE AFTER 14 DAYS.

• I also want to DUO on day 26 as well. If negative, is there a lot of chance of positive on day 28? THAT WILL BE CONCLUSIVE.  THERE ISN'T A SIGNIFICANT STATISTICAL DIFFERENCE BETWEEN 26 AND 28 DAYS.  

• I would like to understand why DUO test in UK and US is considered conclusive given the 99.89% reliability that many experts seem to agree with? IN MEDICINE, YOU NEVER HAVE 100%, AND ANY TEST RESULT THAT HAS A RELAIBILITY OF OVER 99.95 IS CONSIDERED CONCLUSIVE.

• When I ask for a DUO test, should I ask for a specific method or only ask for a 4th generation? YOU NEED TO MAKE SURE THAT THE P24 ANTIGEN IS ALSO TESTED.

• What’s your opinion on my situation?
YOUR ESTIMATED RISK IS LESS THAN 0.1%, ASSUMING THAT THE LADY WAS POSITIVE.  THIS COMBINED WITH THE FACT THAT YOU HAD A NEGATIVE PCR RNA TEST AT 10 DAYS, WE CAN CONCLUDE THAT YOU ARE GOING TO BE ABSOLUTELY FINE.  I AM CONVINCED THAT YOU ARE HIV NEGATIVE.

I did my DUO at day 22. Negative!!! The dr told me that given this result and the PCR, I'm ok and should do an additional DUO at day 28 only for my peace of mind.
