HIV

Dear Dr.

First: I want to tell you what happened to me in detail
- Before 62 hours I have sexual relationship with a woman I do not know she. And used a condom, but a minute later I found the condom on the ground . (I do not remember if practiced with them without a condom or not at that moment).

- Direct after that I put a another condom. But cut off during intercourse the condom .. Then stopped the process of intercourse directly.

- No suck no kiss

Q1. Is there a risk of infection with HIV? ?

Q2. Is there any medication to avoid the disease now?
--------------------------------
- I went directly to an examination of AIDS. The doctor told me you can do after 2 days to 21 days of the communication process. But he advised me that I am doing after 10 days
Q3. Is this true? Examination name (P24Ag)
I live in Kuwait, but the sample will be sent to Germany or france
--------------------------------

- I am married and my wife is traveling and will return after 10 days from now. I am very afraid and I want you to advise me what I am doing step by step
(This means that I want to know the result as soon as possible without telling my wife.)

Q4. Is it possible to give me the names of the tests with the percentage of times work with accuracy.

Q5. Can you advise me to work testing p24ag. According to the doctor's words that we have in Kuwait after a week or 10 days of the incident.

------ Important for me to tell me what to do step by step ASAP

Hello,
Thank you very much for your post.
First of all, I believe that the HIV risk in your case is quite low.  It seems that the penetration was mostly protected, as you are not completely sure if the condom came off or not and for how long.  The most would have probably been a short period of time during which you had been unprotected.  Also the status of the lady is unknown.  Even in the event that the lady was positive, the risk for you would be low, less than 0.1% (less than 1 in 1,000) and that is assuming that you had complete unprotected penetration, whcih is not the case.  Therefore I would not recommend PEP (Post-exposure Prophylaxis) in your case.  However if you really wanted to start this medication (if it is available where you are), you would need to do so within 72 hours of this sexual encounter, and it needs to be taken for 4 weeks, even though I do not advice it for you.
The tests that I would recommend for you would be as follows:
- PCR RNA test after 10 days, reliability approx. 99.89%
- HIV Duo (including p24 Antigen) after 28 days, reliability approx. 99.98% (p24 antigen is already detectable after 14 days, but with a negative result, it cannot be considered conclusive).  It is certainly not reliable at 10 days only.
You need to be tested for the common infections like Chlamydia and Gonorrhoea as they would be much more likely than HIV, though they are all treatable.
In summary, I do not think that you need to panic as your estimated risk is low.  You do not need PEP, and you should have the tests mentioned above at the correct time periods, mainly for your peace of mind.
Best wishes,
Dr José

Thank you for your help Dr.Jose ..
This is the best answer I heard in all locations on the Internet.

After I read your answer immediately contacted the laboratory that I deal with him and asked him about the examination of PCR RNA, who you mentioned .. He told me that this test is available. But it's expensive, almost $ 500 and the result need from 10 days to 15 days. I told him I have no problem in payment. He replied that you can come in any time to do this test. He also said I Can do PCR RNA test befor 10 days.

Q1. I want to ask you about the credibility of this laboratory for  I can make PCR RNA test before 10 Days?

Thank you again.
Honest I have relaxed me a lot .. I am now thinking of not doing any test:)
