Receing oral sex from hiv positive man

Hi doctor.I'm from Jamaica and sometime ago a guy who is hiv positive performed unprotected oral
Sex on me for about twenty minutes.Well I didn't ejaculate in his mouth but I think he had blood in his mouth and after when he told me of his status,he said I couldn't catch hiv from being sucked. But I became sick two weeks after with a sore throat, fever 38 and swollen lymph nodes in my neck and weight loss.I was very worried and was placed on xanax and paxil for anxity. Also my cbc was done 4 weeks after and indicated a reduction in percentage down to 18 percent and nuetrophils increased to 79 percent.Anyway I have done 6 hiv antibody tests since the exposure:

3 weeks rapid and lab elisa

5 week rapid

8 week rapid

10 week lab elisa

14 week lab elisa.

Are these tests conclusive?I don't know if they are third generation tests here in Jamaica. They tested for hiv 1 and2.

¤Thank you for your feedback as I am still worried.

Hello

Thank you for the update on the advent of third generation tests. The situation in the US particularly has been a problem in public health labs because of cost and also due to insurers not wanting to meet the costs of new and better HIV tests.

Third generation tests have been in use in Europe for very many years and only very latterly in the US on a widespread basis. New York has always had a very energetic policy towards testing and has really led the US in this regard.

With regard to your question as to the fingering with pre-cum from an HIV positive male - on paper it represents an apparent risk doesn't it but I've never seen or heard of it being a true threat so I personally would not see testing as an imperative.

best regards, Sean

Hello

Golly we do get the strangest questions!

I've really no idea when 3rd gen tests were "invented" - I've been using them for years - I'm sure you can find the exact date with some research - report back when youve found out.

Regarding being fingered with HIV positive precum.......well, if you are sure that there was a good amount of pre cum from the HIV positive male on his finger when he fingered you then yes, I can accept there is a theoretical risk. I'm not convinced that theoretical risk would translate into a "real" risk in that the amount of HIV in pre-cum is tiny, seems to be relatively inactive and would likely be further deactivated by the cooling/drying effecct as it left his body.

So strictly and reluctantly I would agree there might be a potential risk but in reality I do not believe it.

I hope that helps.

best regards, Sean

Hello

Thank God for your doctor! It's not Dr Anil is it? He was working in one of our teaching hospitals until recently then returned home. I didn't get a chance to say goodbye but he was great.

He's right. You did not need an HIV test. You were never at risk. You will not get HIV through being sucked. You will not.

The tests are fine - negative.

best regards, Sean

I'm sorry I meant my lymphocytes were down to 18 percent and by the way all six hiv tests came back negative and my doctot said I really never needed a test to begin with.

Hi again,Dr Sean.Well I am just reporting my research findings as you asked me to do...

Dr Bernard Branson in the JAIDS Volume 55 Issue pp s102-s105,December 2010,noted that it was during the 1990s that third generation HIV tests were developed that detected immunoglobulin M(THE FIRST ANTIBODY GENERATED IN THE IMMUNE RESPONSE)and did so on average 20-25 days after infection. He added that these tests also incorporated specific antigens to detect both HIV 1 and 2.

Interestingly,he observed that because third generation tests were more expensive than first and second gen tests, they were not widely adopted before the remaining first gen assays were withdrawn from the market in 2007.

Clearly, this would suggest that all HIV tests currently in use are either third or fourth gen tests. This is all very interesting, isn't it,Dr Sean?

Finally,as pertains my last question about fingering with little precvum on finger of hiv positive male, testing isn't necessary then,is it?Please let me know.

Thank you so much and as we say in Jamaica,BIG UP-great work and lots of respect to you!!!

Hi Dr Sean.Thank you very much for your feedback.

By the way,my Dr was not Dr Annil. But glad to hear that you have
Worked with a Jamaican doctor at your clinic.

Now I wanted to ask you if it is necessary to test if someone is fingered
With precum by a person who is hiv positive.

Also when, what year did they invented third generation hiv tests?

I should be happy for your kind response.

Best regards.
