Reliability of test after Insertive Oral with Hiv +

Hi Doctor,

Today I just recived the negative results by HIV-1 DNA by PCR With HIV Antibodies by ICMA which I did 29 days after the exposure of insertive oral sex with a confirmed Hivpositive women.
Questions:

1. How much is reliable this test after 29 days of exposure?

2. Do I need to get tested again (if yes timeframe and what kind of test in US, I dont see any Hiv Duo test in LABCORP website)?

3. Is it safe to have unprotected sex with my wife after this negative results on 29 days?

4. For last six months I had inflamation in skin in penis glans (Swollen)  but not any open sore and also blood in urine. Can this Increase my risk ?

Thank you very much

Dr, Please I dont have access to any doctor until next month, I did this test by order direct to the LAB, Please, can you help me to interpret my results, and answer some of my questions.

HIV 1/0/2 Abs-ICMA
Index value                    result  1.00              reference 1.00      Lab 01

Index value: Specimen reactivity relative to the negative cutoff.

HIV 1/0/2 Abs, qual       Nonreactive 01
HIV 2 Abs, EIA Ratio Negative: 1.00

HSV 1 igG, type spec  result 2.36 High

HSV 2 result 0.91

HbsAg screen HbsAg Confirmation  result Positive Abnormal

Hep B Core Ab, Igm
Hep B core Ab, igm  result Negative

Questiions
1. Is this positive only for HSV 1 and Hep?

2. Four years ago when I donated blood they informed me that I'm carrier of Hep B, that time I used some medicaments for 3 month. Is this positive result for Hep. as a result of that or does it indicate new infection?

3. I never had any cold sore or anything in my mouth or genitals, my wife does have herpes in her mouth maybe once a year. what is your opinion it is old or new infection?

4 Lab wrote that HSV 2 can not be ruled as a posibility for coinfection should I retest for HSV 2? and am I in danger to contract HSV 2 from this exposure?

5.Still six weak after the exposure I still dont have any symptom of HSV, how reliable is this test?

6. HIV test as I understand is Negative after 6 weaks, do you thing this is conclusive or since I have HSV 1 is still need to be tested for HIV later?

7. Since I dont have any symptom of HSV 1, should I start using any medicament?

Thank you very much

Good Morning,

Thanks for your post and questions.

1. How much is reliable this test after 29 days of exposure?

The combination of tests you have had confirm that you are not HIV positive as a result of this exposure. There are no documented or reported cases of people acquiring HIV through being sucked. In fact, the good evidence from studies looking especially at this have shown that there is no risk. So, you didn't have a real exposure and the tests you have had conclusively rule out any HIV from this episode.

2. Do I need to get tested again (if yes timeframe and what kind of test in US, I dont see any Hiv Duo test in LABCORP website)?

Not for HIV. The tests you have had already are more than sufficient to rule out infection from this episode. 4th Generation tests are available in the US - I had thought Labcorp offered them but you may know better than I. I believe Quest Diagnostics also offer them so maybe enquire of them as well. Your doctor should know. In any event, I do not believe that you need further testing.

3. Is it safe to have unprotected sex with my wife after this negative results on 29 days?

From the point of view of HIV you do not have it and so having sex with your wife, if you wish to is fine. However, you go on to ask about other tests and you have had chlamydia and gonorrhoea tesing and these are negative. You might also want to test for syphilis and hepatitis B before you resume sex with your wife to ensure safety as much as possible.. You van do those tests now, at 29 days post event. A hepatitis B antigen is the first marker of Hepatitis B and develops at 2 to four weeks post event. An additional Hepatitis B test whihc is more accurate would be a Hepatitis B PCR.

4. For last six months I had inflamation in skin in penis glans (Swollen)  but not any open sore and also blood in urine. Can this Increase my risk ?

Yes, it can increase your risk, but as I've said above, there appears to be no risk to the person being sucked of acquiring HIV, and in addition, you have already proved the point by having a negative sequence of HIV tests.

Best wishes, Sean
