HIV Risk with Japanese Escort in UK

Dear Doctor,

I need your advise very urgently. A week ago, I had sex with a 20 year old japanese escort in London (U.K). Everything was safe except for a brief slip off of the condom in one position. She immediately stopped, and was not happy about it. I asked her her HIV status and she said she was negative, and was tested monthly. I contacted the owner of the escort agency and he told me there was nothing to worry, all his girls were tested every month. I also learnt that she is a student.

What is my risk of an HIV infection? She did take extraordinary level or precautions, only brief kissing, no french kissing, and no touching her vagina without a condom on the finger.

I am greatly troubled that I may have contracted HIV, and I am hysterical. It as begun to affect my work. I tried to find out about testing options and learnt about something called HIV proviral which the clinic says can be done after 10 days with an accuracy of 99%, is this true?

Please advise, I do not  know who to talk to, and could really use your help to put my mind at ease.

Please, please reply.

Regards, Abby.

From your description, it sounds like your CSW partner almost certainly is not infected with HIV.  In general, escorts are at greater risk from their customers than the other way around; i.e., she was smart to be concerned about the condom, for fear that you might have infected her.  In other words, her reaction to the apparent condom failure should be reassuring to you.  It means she isn't infected and wants to stay that way.

I see no serious concern and do not recommend you be tested for HIV.

Best wishes---  HHH, MD
