Hiv Exposure after Hand Job with ejeculation

Dear Doctor:
Some 6 days ago, I went to massage parlor here in China. Place was reputable and looked fine. I came with just massage in mind and asked Aroma Therapy- Oil massage.  My masseur was Chinese girl completely dressed and started giving me massage . Some 30 minutes later she has asked me if I want to have massage of my front body part in area of my pennies. I made a mistake and said yes.  At that point she massaged me for about 5- 7 minutes with both hands,  a lot of oil, touching my pennies head top and rest, and occasionally touching my anus “ring” . I was not thinking about anything. I ejaculated in her hands and she quickly went and gave me towel to clean. This was extent of my exposure, I had no Vaginal, Oral, Anal intercourse, I have not touched here body at all. I had no visible wounds on my pennis nor visible blood anywhere on my body.
I did not necessarily inspected here hands imidatly – because I did not know to what extent I will freak out – after reading what can I get. I went back 3 days after to check on her hands and found nothing. I learned that this girl occasionally gives this types of massages and she claimed to be in good health . Being in china that word does not mean much.
1) Am I in any danger of getting HIV ?
2) Do I need to get tested?
3) Is bodily fluid contact considered when I ejaculate and touch someone’s hand during masturbation ? If someone hand hypothetically has open wound can blood flow back to my penis through my semen?
4) I am still in China and planning to go back to US in a month. I am married man and have great sense of guilt on what I have done and how much I disappointed myself and my wife. I am trying to deal with this, but my day starts and ends with this incident on my mind.  I am concerned if I can practice sex as usual with my wife ? Last thing I want to do is to put here in any risk from getting anything.

Your fears are groundless.  There is no risk to you from receipt of masturbation and have no reason to worry.  To specifically answer your questions:

1.  No, no risk at all.
2.  No, you do not need testing
3.  Hypothetical questions are a waste of time and will not be entertained.  Masturbation is safe sex with no risk of HIV transmission despite the fact that it is usual for body fluids are typically spread from person to person in the process of masturbation by another person.
4.  There is no reason for you to worry about risk to your wife because of the events you describe.  Sometimes discussing things with a counselor, if you are not comfortable talking with your wife can help to move on from the sort of guilt you are describing.

Counseling is confidential.  My advice wold be to seek the care of a counselor.  You might call someone at a health department near where you are staying and ask if they can tell you where you might find a counselor.

Dear Doctor:

I apologize for bothering you again. I am writing after almost a month since my last post and just wanted to consult one more time with you.
I thought that my worries are going to go away since your last post - and they did for some time, but later instead, I became scared and sometimes desperate. I know that you have told me I am safe and I believe in that, but my mind is making me go around and around. I fill like I have compromised my whole life with this incident and that if I make love with my wife I will ruin her life ( I am currently in US and have been intimate with my wife but trying to stay away ). I also closely watch my body and my health and sometimes I identify certain things as symptoms- like half of dozen pimples form sweating on my shoulder or stiff neck that started this week. It blows my mind. I go to this post you posted last time and I read it over and over again and tell to myself I am fine, I don’t have to worry. I wont to believe in that, but instead I fill depressed and consumed with guilt .
I have visited International Expatriate Physician Group in China and talk to my physician there twice, and he told me that I have nothing to worry about – I did not had sex. Is there anything you can advise me to do. You have mentioned counseling earlier - do you have any recommendations with CDC maybe. Is counseling on this subject confidential?
Please advice. I know that you are busy with far more complex cases but I would greatly appreciate it if you can take a moment one more time.

Dear Doctor Hook:

I am reading your respons to my #3 question,  and just so that I am sure I want to make sure you understand:

She has been masaging my penis until ejeculation and I ejeculated while she had my penis in here hands. She was doing all. Not me. When I said I have not touch here, that measn I have not done anything else, she has clearly masaginign me and doing this.
Is this still considered safe?
