confused and scared

Dear Doctors,

I’m 41yo male from Western Europe. I was aware that masturbation is a safe sex. Therefore in my sexual life (although not so active) outside of my family I limited myself to the mutual masturbation activities only. No kissing, no oral, no penetrations. I asked them if they had hiv and they told me not.

About 9 weeks ago I had mutual masturbation with two (not at the same time) different white males ~30yo from Germany. They massaged my genitals and I massaged their till ejaculation. They did not use their secretions for lubrication but I cannot exclude that their hands transferred some of their liquids onto my genitals. By that time I had some balanitis on the rim of the penis head. But not bleeding, not open I think. I also think that my genitals were not in direct contact with their genitals.

At week 5 after this encounters I started to develop muscle pain and weakness and sensitive nerves in my head, and in 2 days paraesthesia in my hands and later in my feet, numbness in the coccyx area. Plus the fever went up to 38.5C. I had some really mild running nose and cough. The fever was gone in 5 days, but the neurological symptoms are still present (5 weeks after the onset). I went to neurologists, one of them suspects some light form of GB, the other thinks it is related to functional disorder. I explained them my sexual encounter and they did not proposed me hiv test.

I am so scared and all the time thinking about hiv as a reason for my symptoms. I had a negative test 1 year ago and since then these were only sexual encounters I had outside of my family.

So, today I went to anonymous testing center and the nurse who took my blood told me that my risk was not zero. I’m very scared because this goes in contradiction to what i knew about hiv. The result will be ready next week Tuesday and I am scared to go there again. I cannot bear it.

In your experience, what do you think my chances are? Thank you for your help.

Dear Doctor,

Is there any other "sort" of mutual masturbation, which is more risky?

Mutual masturbation is safe sex even if you have cuts on your penis or balanitis.

One more question, sorry. In one of the questions on this forum I read a comment from H.H.H.:

...I would also encourage you to plan on having a more specific conversation than "do you have anything?"  Ask specifically about HIV status and avoid sex -- even protected or otherwise safe -- with those who are positive (and not on anti-HIV therapy), don't know, or seem evasive about it.

Does this advice mean that even safe sex with an HIV infected person is not a zero risk?

Thank you for your reassurance and a word of confidence.

Do I understand correctly that the nurse was over-conservative or simply wrong?

Is it no risk to continue having sex w/o a condom with my wife?

And one more question. I am quite nervous and anxious person. Every time I have a sex relation I regret and get scared of HIV even though I am practicing safe sex. One way would be not to have any sex outside of my family to be absolutely sure there will not be any infection. Or maybe I should use a condom for mutual masturbation. What would be your advice?

Welcome to the Forum.  Let me start by congratulating you on your commitment to safe sex.  I can assure you that all the steps you describe, such as discussing their status before sex and only engaging in mutual masturbation without penetrative sex of any sort will prevent you from getting HIV.  I am confident, as are the other health care providers that you have spoken with, that you will not have gotten HIV from the activities you describe.  Your symptoms are not due to HIV acquired through the activities you have described.

Please understand that the symptoms you are having are not due to HIV but some other process of which there are many possibilities.  I am confident that the HIV test you have taken will prove that you do not have HIV.  when you get that result, I recommend that you continue to work with your doctor to determine what is causing your problems.  EWH

One more comment. A week later after the onset of my symptoms, my family members had also high fever and flue like symptoms.
