penile lesions after insertive fellatio and protected penile vaginal intercourse

unprotected oral sex, protected (lube condom) penile vaginal sex and penile lesions

Here  goes.  I had unprotected insertive fellatio and protected penile vaginal sex with a sex worker in Bangkok, Thailand.  She used lube for the condom.  Not sure if it was oil or water based lube. The condom didn't break. 3 days after the encounter I noticed small scab lesions, fissures (0.3 cm) on the glans of my penis. Approximately 10 lesions.  They weren't group together but randomly noted on the glans.  No pain, mild discomfort, no dysuria, no discharge, no fever, no lympadenopathy, no drainage.  At the initial presence of the lesions I started valtrex right away.

The lesions lasted 6 days and went away on day 9 after encounter.  It has been a total of 12 days since.   I  have no current symptoms now besides anxiety and insomnia.  My questions are as follow:

1. HIV. Am I at risk for HIV?

2.  Does the lube increase my chance of contracting HIV if it is oil?

3.  In Thailand, I believe HIV subtype E is more pronounced for heterosexual intercourse and easier to contract, would that impact my chances here?

4.  Can the presence of penile lesions 3 days after intercourse represent a portal of entry for HIV?   I read where mucocutaneous lesions can be a symptom of HIV.

5.  Chancroid, syphilis?  The lesions were never painful, no vesicles, no adenopathy, no malaise, no myalgias, no fever, no discharge.  However I did have discomfort

6. When should I get tested for all std, herpes, hiv, syphilis?  I'm at day 12.

7. I can post photo if helpful.  Need to know where to post.  Thanks Doctor Jose

Please help Doc.  I feel so stupid and need to find some resolve.  Thanks. Worried beyond

Sorry - we had to remove the photo.  Genital pics, even for clinical use, are not allowed.  We are a PG-13 site.

Sorry for the inconvenience.  If you want to link them on another site, that's fine.

Dr.  Jose I can't tell you how much your words have helped to calm me.  I have learned an incredible valuable lesson here.  Thank you.  I uploaded the photo.  If you look at the opening of the urethra you will see some excoriated/scab lesions.  There are about 6 maybe 7 present there.  Also, along the rim of the glans (proximal to the beginning of th shaft), from the center to your left your will see ulcerated/scab lesions.  Thoughts

Hello there,
Let me answer to your questions one by one, as follows:
1.) Not at all.
2.) It would increase the chances of the condom breaking.  However if this did not happen, no need to worry.
3.) Not at all. No risk from receiving a blow job, no risk from protected insertive vaginal.
4.) Not at all.  The lesions appeared afterwards, which might have been contracted through the oral sex received.
5.) It seems to be too early for a syphilitic chancre to appear.  If not painful, unlikely to be herpes.  The lesions seems to be more consistent with balanitis, either fungal or bacterial.  Difficult to diagnose without examining you.  I advice you to see a doctor and get appropriate advice.
6.) If you still have symptoms, I would advice you to be tested for Chlamydia, Gonorrhoea, NSU, Thrush, etc now.  You might need a urine test and a swab.  Also you might want to have a PCR swab of the lesions for herpes and for syphilis, in order to exclude this possibility.  Otherwise you would need to wait until day 28 to have a HIV Duo and a syphilis serology.
7.) You can do it on any photo website and post here the link.  Otherwise you can always use the photos application here at Medhelp.
Best wishes,
Dr Jose
