PLEASE ELP

I had unprotected oral sex with in a massage parlour in  dubai.
One one month after this exposure i had a unprotected vaginal sex due to condom break.
Conducted a HIV PCR after two months of oral exposure & one month of vaginal exposure with result of viral load less then 20 & CD4 count 600.
Conducted a screening test (may be TRI-DOT don't know exactly which test is used for HIV screening) after three months of vaginal exposure.
I had some symptoms like high fever, chills after one week of vaginal exposure & again after 2 weeks of vaginal exposure.
After one month of vaginal exposure i had a severe back pain, so severe that i could not move. i also had some joint pain & body ache.
i never had diarrhea, headeches, reahses etc. except for few red rashes on legs , chest & face after 3 months of vaginal exposure.
please advise how accurate these test are which i coducted , can i believe & be at peace conidering i dont have HIV. Do i need further testing or the screening & HIV PCR are reliable.

Dear Elia,

I don't have any further comments to make for you. I think you're fine.

I've repeated my last comment again beneath for you.

"Hello Elia,

I think from what you say - and assuming that the laboratory has correctly performed the tests, particularly the PCR test, then you can rest assurred that you are not HIV positive. I'm not familiar with the Tri-Dot as it seems to be a test manufactured and distributed within India. I did find a comparison of the Tri-Dot against a 4th generation test which implies that the TriDot is a modern third generation test. As such it should give you greater than 99% accuracy at 6 weeks after any exposure. The combination of TriDot and PCR should rule out HIV infection.

You can read about the TriDot comparison by pasting this address into your browser:-

Thank you very much Dr. Sean for your reply.
kindly note that PCR was done 40 days from probable exposure & HIV screening (may be TRI-DOT or other test don't know which test but they gave result in 40 minutes was done 90 days after the probable exposure.
PCR was done through COBAS TAQMAN as mentined in my above postings.
Please advise how reliable these tests are?
can i rely on these tests & get rid of this HIV stigma.

As Dr Sean Cummings said, the PCR test for HIV that you took is highly likely to be correct. The PCR test result is also supported by the results of your screening test.  You should not be worried about HIV anymore.   Since you took the screening test 3 months after the exposure you are certainly HIV free.

Please see my PCR RNA test results done after one month of probablbe exposure:-
HIV-1 Viral load by COBAS TAQMAN :- < 20 copy/ml
%CD3+/CD5+ (T CELLS) :- 66.1 %
ABSOLUTE CD3 + LYMPHOCYTES COUNT:- 893
%CD3+/CD4+ (T HELPER CELLS) :- 41.7 %
ABSOLUTE CD4 + LYMPHOCYTES COUNT:- 564 /ul
%CD3+/CD8+ (T SUPRESSOR CELLS):-18.5 %
ABSOLUTE CD8 + LYMPHOCYTES COUNT:- 250 /ul
CD4/CD8 RATIO : 2.25
ESR - 20
I don't know how to interpret HIV PCR results whether positive or negative,
my three months post exposure HIV screening test (dont know the exact method - may be TRI-DOT) was also negative.

I did the PCR RNA test in India from a well known international laboratory.
I have already done screening test (may be TRI-DOT- don't know the exact method) after 3 months of probable exposure.
Is these tests are sufficient or i need further testing.

Hi Elia

The HIV RNA is very likely to be correct but since I don't know where you are, which country or which laboratory or indeed which test I would strongly suggest you have a HIV DUO or HIV COMBO test which will look for HIV 1 and 2 antibodies and also p24 antigen.

Best regards, Sean
