Need advice please

Dear Doctor

39 days ago i met a sex worker and received unprotected oral sex from her to point of ejaculation in her mouth, performed fingering on her (minor cut on my finger from day before (hangnail that i pulled and bleed)) and lots of kissing with tongue.

I attended a STD clinic on day 14 and had all the tests done with the exception of HIV. The nurse gave me a 1 time high dose of antibiotics to clear Chlamydia (precautionary). I also attended my doctor as was also having lower abdominal pains and he prescribed Ciprofloxacin 500mg for 7 days.

Since this I have had flu like symptoms 12 days (excessive tiredness especially after eating where I just crash), first day vomited once and loose bowel movements for 2 days, ulcer in mouth, thrush on tongue, anus and genitals that I cant shake (been there for 12 -14 days), 4 hard red blood spots that are isolated and appeared on my chest, 2 itchy rashes (very minor) on chest, weakness in backs of legs- comes and goes on a daily basis. Today I have had blurred vision and find myself having to really focus to get control back (blurred vision lasts for around 5 minutes at a time- noticeable when reading (I have never had this before)).

I have been prescribed Fluconazole 100mg for 14 days to get rid of the thrush and am on day 7- this isn't working as still have very itchy anus and white tongue but haven't disclosed to doctor the above as is a friend of family and wife uses him also.

On day 30 I had a RNA HIV1 test run and it came back as non reactive.

The questions I have are:
1. Are the symptoms that I am experiencing conclusive of potential HIV infection?
2. How reliable is the RNA test that I had run on day 30?
3. I am getting anxious as my wife is becoming very suspicious and wants to know why I wont sleep with her- when is it safe to do so?
4. Is there anything I can do to get a conclusive result prior to the suggested 90 day window?

You can - and should relax about HIV.

You will not acquire HIV through being sucked.

The HIV test you had done is reassuring for you but for you to be totally convinced I would personally have recommended a 4th generation HIV DUO test using antibody and antigen.

I suspect that what has happened is you have become very anxious about the prospect of HIV transfer to you and are seeing things of no real relevance or importance. Thrush is common and has a large number of causes with HIV being relevant typically several years down the line.

The only thing I would recommend is to have an HIV DUO - or COMBO and that will reassure you hopefully that you are not HIV positive from this.

Ok status update - after confirming with the sex worker she has advised that she has run a panel test and is HIV+.
As you can imagine I am now really anxious. I am now skeptical of the RNA test result - was only for HiV 1 and am showing all symptoms that are conducive of infection (had flu like symptoms lasted 10 days and now have excessive thrush in all oraphis extreme itching in anus, thick white coating on tongue, pins and needles in hands and feet and weakness in backs of leg muscles only).
Please could you answer my previous questions with consideration to this update? Thank you in advance for your vigilance.

This thrush is getting worse my anus feels like its on fire and itching, I have constant upset stomach and flatulence, the drugs aren't working- I have heard this is a direct sign of ARS- is it?

Forgot to mention that I have also loss of appetite and am around 10lbs down over last 10 days
