Like I say ive probably had 12 other riskier than normal partners since, lap dancers sex workers etc, but always have used a condom, and never noticed a break. As was the case 5 years ago the condom appear in tact and was still tight on my penis so I presume had it broken I would definitely noticed.

Welcome back to the Forum. I'll try to help. I worry that you are associating two unrelated events. Condom protected sex is safe sex. You have told me that your condoms did not break and I agree- if they seemed intact, they probably were. Therefore there should be no appreciable risk for STI of any sort, including HIV.
There are NO STIs which cause testicle shrinkage, less firm erections or the sorts of lumps you describe. My advice would be to address the concerns you have noted above with your doctor but not to worry that they might be caused by STIs of any sort. In my opinion, there is no need whatsoever for STI testing at this time.
