Correct use of condom ... scared of hiv

Hi folks ... really sorry to bother you with this question. I had protected sex with CSW and found that after sex, my condom was inch down from the base of penis. Had some vaginal fluids around the base of penis. Also the condom rolled out easily from the penis while removing it. Should I be worried if I used the condom safely?
