Doc Need your advise and help

46 Year old white straight non IV drug using male. Last tested August 2007 Neg.  Had 3 one time sexual female partners. When I say one time I mean we spent night together and had sex a couple of time durring the night. but only that one time. All 3 of my partners we non sex workers.  all 3 were the same type of exposures. Lots of manual stimulation then they preform unprotected oral on me then I place a new condom on each time have vaginal or anal for a few minutes then take condom off and finish with them doing oral on me.  I never noticed a condom fail. I was never drunk and I hope I would have noticed. I once placed my hand inside a womans vagina and kept it there a few seconds when I took my hand out I did not notice any blood, only lube and fluids then washed hands. Now here is the thing . I was thinking I was having totaly safe sex. But I got nevous when I found out 48 yo school teacher I was with had a very wild past that included going bareback. She tole me she was neg and we USED a condom. But who knows if she is lieing. My last sexual encounter was June 08.  I have had no symptoms no sickness besides 24 hour flu  one time in december.  But last week I noticed a gland or a node on the right side of my neck was swollen and hurts when I touch it.  Not sure if its a gland or a node, its under my chin below my jaw size of a pee.. I also have developed a fever sore on my lip, I have never had a sore even with the flu this is my first time ever.  I know its been  8 months but could my exposures have somthing to do with my swollen node and sore on my lip and could it be late ARS. I thought I was having safe sex.  But now I am going crazy thinking I did somthing wrong.  Does my exposures require testing? I feel warm all the time but when I check my temp its always under 100. and I have no other swollen nodes on my body that I can feel. COuld I be seroconverting. Please help.  thanks for your time ....

To yazzinnyc, no, tha thtey are caker sores does not conecern me in the least. There is no change to my assessment or recommendations

Relax those cranker sores are from Stress and Anxiety your best bet is to try and refrain from searching on HIV symptoms. Dr Hook has reassured you that you do not have HIV. Dont let the fear take over your life trust me it will get even worse if you do. Goodluck I am here if you need me pm me anytime..

DOc sorry to come back so soon.  But I just wanted to correct something.. I do not have a fever sore  it is a canker sore it is on the inside of my lip and is healing but I feel another one coming right below this one.  ANd as I said I am in my 40's and never had one before.  is this somthing I should be worried about..  Last post !

Without examining you it is hard to say what is going on.  A cold sore is a possibility however bacterial skin infections (pimples) are common as well.  I can assure thta whatever is happening on your lip is not related to HIV (which you do not have).  EWH

Thanks doc you are very wise to note the anxiety issue even though I didnt mention it. I am not going to get into the long what ifs, and how abouts and make this a never ending thread.  one simple question and I am outta here.  I have never in my life had a lip sore and this one is growing like a fat lip and there is a small white soft area, can  it just be a fever sore, but I have never had one and At 46 years old think its kinda strange to have one now..  and thanks again....

Congratulations on your commitment to safe sex.  It makes my job on the forum easier because I can emphatically tell you that your risk for HIV is VERY, VERY low.  You used a condom for all vaginal and rectal sex and condoms work.  They protect you from any STDs your partners might have had, including HIV.  As for oral sex this too is close to no risk.  Estimates are that IF one of your partners were infected with HIV (and this is unlikely, even the "wilder" partners), your risk of getting HIV is no greater than 1 infection per 10,000 exposures and many experts believe oral sex is no risk at all.  As for masturbation, no risk.

Swollen lymph nodes are non-specific and do not mean that you have HIV, if in fact you have a swollen node at all.  

Do you require test.  No, but I would encourage you to be tested.  You describe unprotected sex with at least 3 partners.  Thus you qualify for periodic HIV and STD "screening" (testing in the absence of symptoms) because you have had multiple partners over a period of less than a year.  As a matter of personal protection, we recommend annual testing for anyone who has had two or more sex partners in the past year (we consider this to be health maintenance- we also recommend you get your blood pressure and cholesterol checked regularly).  When you test is negative it will go a long way towards resolving the anxiety I sense you are having related to this exposure.
