Just A historical question about protected sex

For protected sex, which I mean sex with the whole condom being correctly used and without breakage throughout the vagina sexual intercourse. Does it means that never have anyone got HIV infected with the above situation?
And secondly, is it possible for the HIV or Hepatitis B or C being infected through passing through the condom particle with out any appearance breakage of it?

In some of your past comment, you present that if consistently and correctly use condom can only very highly effective to prevent hiv or hepatitis. But why now you can be so sure with answer no? So you mean this no by 100% sure? or just like 99.99% sure? And also why some reference said it only takes 80% effective for condom? thank you for your reply and it may educate me alot if you may answer it! Thx!
