Need help on possible risk with a condom

Hello I had sex with my ex boyfriend and we always used condoms. However I still get this "what if" in the back of my head. On the net, there are a lot of talk about tiny holes that are in condoms. Is this possible?? An I 100% safe if I used one?? And how do I know if it breaks??

A lot of my friends tells me that I would certainly know but I want to make sure. I am also afraid of doctors... Should go still get screened for HIV even if I used a condom
