Possible Exposure

   I am a male who had sex with another man that I’ve been seeing for the last month as the receptive partner. We’ve had sex once before and he used a condom the whole time and he has assured me that he’s been on PReP since July 2017.  We’ve talked about it at length and he even described how he started PRep and where he gets the PRep from.

  Yesterday,  we had sex and I was receptive for at least 10 minutes with a condom and I told him I needed a break.  After 5 minutes I wasn’t sure I was ready to start again so he said that he would stick his penis in me quickly to see if I’d be ready.  He stuck his penis in me without a condom for at least 5 seconds and took it out when I said I was ready so he put a condom back on so we had sex and finished.  

  I’m not sure if the 5 seconds warrants a risk where I should seek PEP.  He assured me he’s on PRep and has recommended that I start PRep.  I know that there is a risk for HIV because it was brief unprotected sex and even though the guy I had sex with assures me he’s on PRep and negative, you never know.  Please advise.

So I retalked to this guy and he wants to be in a serious relationship, so he wanted to be totally honest.  He confirmed that he is negative and that his ex was actually his husband who he is in the process of getting a divorce.  He let me know his ex was hiv positive and that is why he started PReP.  He’s actually been on PReP for 4 years.  He told me that he’s been separated from his husband since July 2017 and his last  hiv test came back negative in December 2017.  I told him about my anxiety about this and he agreed to take an hiv and std test with me this weekend.  He confirmed that he had condomless sex with his husband all the time and that he always tested negative for HIV.  He seems very confident that he doesn’t have HIV, so I’m hoping the brief insertion won’t be an issue.

He went to get tested at a local clinic in July 2017 and they let him know he qualified for free PReP, so he decided to start for the added protection.

He last was tested for hiv in December 2017 and was negative.  He confirmed his last unprotected sex was with his ex boyfriend who he broke up with in July 2017.  He started prep after he broke up with his bf and has not had anal sex since but has had oral sex with two other guys

Ok, that changes everything. Any test is conclusive after 3 months so he is negative and you don't need to take PEP or test. Oral isn't a risk.  

BtW, you might inquire why was/is he taking prep since July other than for this instance with you 9 months later?
THere is a tiny risk that he couldn't get valid December test results while on prep though due to delay,
However my understanding is he had to be tested negative before they gave him the prep, so he should be ok, other than if there was a transmission during his window prior to being tested.
Maybe someone else can comment but your risks seem small and taking PEP has its own permanent dangers.

You have to make that decision. Were you able to figure whether he is conclusively negative, or has he had unprotected anal exposures since his previous test window started? Oral is not a risk for HIV. fyi.

Just wanted to add that he probably wasn’t inside me for more than 2 seconds and he confirmed that he was tested negative for stds and hiv in December 2017

A duo test is conclusive after 28 days and any other after 3 months, so you have to know when his last unprotected was in order to know if his test proved he is negative.
Any unprotected penetrating anal is a risk - unquantifiable - so only a test can determine your status if he is not proven negative.
