Symptoms?

I hooked up with a prostitute who turned about to be a shemale. I was a bit drunk and curious.  We mutually masturbated for a few minutes lying down. At one point I noticed a small patch (size of a quarter) of *** on my inner leg.  It was on intact skin with no lesions or cuts, etc.  I don’t know if this may have been my pre-*** or if he came and I didn’t realize it.  I may have also seen a bit of wetness on the head of my penis.  Again could have been my *** or his.  That was the total exposure.  No oral sex, no insertive sex at all.  My concern is if he came and my penis rubbed up against the *** possibly exposing my urethra to his ***.  Two days later I was treated for a sinus infection and was prescribed 875mg augmentin 2x day for 7 days. Exactly 5 days after the exposure I noted some mild burning and pain at the tip of my penis.  I have a history of prostatitis and I believe that's what this was (perineum pain, mild tip of penis pain). No pain on urination and no discharge.  The symptoms resolved after about 10 days.  Then exactly 21 days after my exposure incident I developed swollen lymph nodes in my neck, groin, clavicle, and possibly my underarms.  The clavicle and underarms were mild and only the cervical and inguinal nodes now seem enlarged. These have lasted for 4-5 days.  I have had no fever, no sore throat, no rash, no GI symptoms.  The only other symptom is my lower back aches.  Note my father had hodgkin's at age 38, I'm 44.  The symptoms have been non limiting, in fact I ran 6 miles yesterday.  1) Do you see any exposure to STD’s/HIV? 2) Would the augmentin have been effective on gonorrhea? 3) Do the generalized swollen lymph nodes in the absence of other symptoms argue against HIV infection?  And as I have had these swellings for 4-5 days wouldn't any other symptoms have arisen by now?  4) Would inquinal lymphadenopathy or other lymphadenopathy cause the lower back pain?

Dr H, I don't want to belabor the point as I know you are extremely busy, but I wanted to be sure you noted that my concern was the possibility of exposure if the tip of my penis came in contact with his semen.  The wet spot on my leg I was not too concerned about because of all I've read HIV cannot penetrate intact skin.  My worry was that somehow I rubbed into his emission with the top of my penis.  If I did, do you still feel my exposure was non-existent?  I know its my anxiety getting the best of me but hearing from you is very reassuring as I'm sure everyone on the forum can attest.

Thanks so much for your help Doc.  The anxiety has been overwhelming.  Last night I took a xanax and finally was able to relieve some of my anxiety and got a good nights sleep (I hadn't slept more than 3-4 hours for about 3 days due to the anxiety).  After lasts nights rest of 8-9 hours I woke up feeling much much better.  My back pain is much reduced and although I still have some mild cervical node feelings I'm almost back to normal.  

I appreciate your help tremendously.  Unfortunately sometimes its things like this that make you realize how much you love your wife and family.

HIV is not transmitted sexually except someone's penis enters a body orifice, primarily the vagina or rectum, very rarely the mouth.  Same for gonorrhea and other STDs.  You didn't do those things, so you were not at risk for HIV or other STDs.  You cannot get HIV or other STDs by mutual masturbation or by getting infected secretions on your skin.

As I have said innumerable times on this forum, symptoms never are a useful indicator for or against new HIV infection, because even the classical symptoms of HIV are caused much more commonly by other viruses and illnesses.  (Read just about any 10 threads at random for more discussion.)  Anyway, your symptoms don't suggest HIV (see below).  You're probably a better judge than I am whether your genital area symptoms are due to a recurrence of your prostate problem.

To the specific questions:

1) No exposure.  2) Gonorrhea isn't possible in this situation.  But in direct reply, Augmentin would probably work about 80% of the time. 3) Self-assessment that one's lymph nodes "seem enlarged" is very unreliable; most people who come to me with that complain don't have anything abnormal that I can feel.  But you are right:  even if enlarged, that alone without other sympoms doesn't suggest HIV.  4) lymphadenopathy doesn't cause back pain.  Sometimes problem deep in the back of the abdomen can do it, and that can inlude inflamed intraabdominal lymph nodes.  But that's really a stretch.  Most back pain is just back pain.

Bottom line:  Of course see a health care provider if your symptoms continue or you otherwise remain concerned.  But I see no concerns here about HIV or other STDs.
