risks

What risk would there be in kissing someone (exchange of saliva) and that person using saliva on your penis as lubricant during masuturbation, saying the other person was hiv positive ?

If someone presents with a an hiv risk, surely as soon as they inform a doctor that they had unprotected sex for example, the fact that they may have excnaged saliva before-hand would not even be exmained ! Therefore how do we know saliva isnt a risk ?
