second HIV exposure while on pep

i recently got into a relationship with a lady i did not know of her HIV status. we eventually had an vaginal sex (with no condom). i was very worried though she told me not to that she was sure that she is negative. but out of fear, i went to my clinic and i was given PEP treatment at about 40 hrs post exposure.
the relationship continued and we had another sex 6 days after i started PEP. i was not a brief sex at all and i ended up having some injury on my penis as a result of the activity.
I almost discontinued the PEP, but asked her to just do the HIV test, and it came out positive. i was shocked!!!!!
i have not had any rashes, fever, sore throat and all that except jaundice, nausea and severe weakness once i take the drug.
i experienced an unexplained burning sensation,needle like pricks and colicky pain in and on my penis and my scrotum. sometimes i feel like something wants to come out but nothing comes out of my penis. this started about two weeks after exposure. but no discharge nor sore nor rash around my perineum nor inguinal area.
all STI screening i did were negative and i had to take treatment for STI just in case ( this was my decision).
i have completed my pep and got tested again at day 33 post exposure and day 30 on pep and im still negative.
i was placed on LAMIVUDINE AND TENOFOVIR + ATAZANAVIR AND RITONAVIR.
my questions are
1) will my second exposure limit the efficacy of PEP?
2) should i have extended my PEP to 6 more days.?
3) are those symptoms i had possibly due to seroconversion?
4) is my current negative result encouraging?

Every postive antibody test has to be confirmed with a Westernblot test.  Was a WB done for confirmation?

Also,  as a standard protocol,  you need to wait for at least 28 days post finishing PEP for a conclusive with a IV gen HIV Test or 6 week with Antibody tests of III or any lower generation.

Your second incident did widen the bracket for exposure but being on PEP at that point was also something that was working in your favour.  However,  the idea of working up odds is not something that can give you any answers here,  the right course is to wait and get the right test at the right time as stated above for your reference.

Your doctor should have extended pep 6 more days since you found out your partner is indeed positive. Test 4 weeks after your last dose of pep for an extremely good indicator of your status.

Also. You need to get tested now for other STDs like chlamydia and gonorrhea.
It is possible that she had a false positive for HIV so you have to know which tests she took.

im sincerely grateful to all that have imputed their expert advice regarding my questions.
I am two months post pep now and tested negative with the anti-bodies test.
I still feel extremely tired and can not even walk at times, I feel excruciating pain at my groin, feel aches in my joint and bones. my palms and sore of my feet are peeling. I have severe headache.
please I would like to know if this could be sign of ARS or possibly an STI.
I have done STI screening two months ago which came out negative.
I sincerely Don't know what to do now.
the last three months have been a living hell.
You have to wait 12 weeks for antibody test so you wasted your time. You can do a duo now for a conclusive result because that only needs 4 weeks. Your symptoms can't prove anything (and might be imaginary but that is another issue) so you need to stop wasting your time analyzing your body. No one here pays attention to your symptoms , fyi.
ARS symptoms do not last 3 months.
Symptoms are meaningless - some people are asymptomatic- so it is a waste of time discussing what he thinks are symptoms.

Thanks a lot for your response.  I am truly grateful.
I asked my doctor if I could extend the duration of pep, but she said no and refused to prescribe more med.
however, right now I have had no rash, fever, nor sore throat. but I have been having pain underneath my right chin and neck.
I cant palpate any lymph nodes but I am sure something is going on in my nodes. I can feel it. it hurts when swallow especially in a particular position, but I can tell its not in my throat.

I also feel pain in both my armpit. worst in my left. I still cannot palpate the nodes but it hurts.
im constantly tired with no appetite as well.
im currently on only doxycycline for ? chlamydia.
please I would like to know:


1) could these be early symptoms of HIV seroconversion?

2) could it be the side effects of the PEP I ended 10 days ago?


3) could it be from the doxycycline im using.


4) can seroconversion occur with just only sored lymph nodes and no other symptoms?
