Is your Doctor aware that you have pains in your arms at rest? and that nitrates remove the pains?

I take isosorbide, a nitrate, before engaging in my exercise routine at the gym.  It is a slow acting nitrate (last hours) as opposed sublinqual nitro (lasts about 20 minutes).

Your echo indicates a strong heart functionality evidenced with an EF 68% (normal is 55 to 75%).  Your heart is structually normal valves and heart chamber and wall size is acceptable.

You didn't include stress test and ct angiogram results.  One can have chest pains relieved by a nitrate without ischemia (occluded vessels) or a heart problem.

I'm assuming by "angised tablets" you mean nitroglycerin tablets.

One thing you may want to try before you engage in any strenuous activity is to place a tablet under you tongue BEFORE you engage in the activity.  When I started on an exercise routine, years ago, I complained to my cardiologist that the angina I was having was horrible during and after the exercising.  He suggested I use my nitroclycerin spray (I've never used tablets, have always used the spray) just prior to engaging in the activity.

Surprisingly, it worked.  I continue to use my nitroclycerin spray just prior to engaging in any strenuous activities, and I have never had any further angina problems since.

Nitroglycerin dilates your blood vessels and therefore lowers your  blood pressure and strain on the heart. But most importantly, it dilates the coronary arteries, when your coronary arteries are having problems dilating, it causes the angina. When your coronary arteries are dilated, that means they are openning up and allowing better blood flow.
