Heart Disease and Heart Attack

My partner just underwent quadruple bypass surgery. One of our close friends just had a heart attack. I am wondering the differences between heart disease and having a heart attack. What is the difference between the care ie elliminating salt from your diet or fats. Both men have received different advise as far as diet and excercise go and I don't understand why. Also why would there not be follow up treatment to access if heart disease is present in our friend who had the heart attack?
