LVEF=20%

My 2DECH reportConclusion is:
> Ischemic Cardiomyopathy
> Dialated LV.
> RWMA : All Wall of LV are Significantly Htpokinetic with Virtual
                 Akinesia of Anterior Wall & Anterior Septum.
> Deoressed LV Function. LVEF = 20%
> Grade I Diastolic Dysfunction
> Severe Mitral Regurgitation.
> Severe Tricuspid Regurgitation.

Please guide me .
What is my condition exactly.

It can be either way, but in this case I think the valves are distorted due to enlargement of the heart, due to ischemia.

So do you think the valve issue is a result of the enlarge LV or the LV is the result of the internal pressures from the regurgitation? I have not though of it your way, makes sense as well.

The diagnosis is ischemic cardiomyopathy which basically means that coronary artery disease has caused problems with your heart. You must have significant blockages and they must have existed for some time now. Have you not had symptoms and been to the hospital before?
I think that the enlarging of the heart has caused the valve orifices to mishape, causing them to severely leak. I have to be honest and say I'm surprised you are getting as much as 20%. It doesn't mention your right ventricle, but with your tricuspid valve regurgitating too, I would hazard a guess and say the right ventricle is also enlarging. Judging by the septum and anterior LV wall motion, it sounds like your LAD is in very poor shape and needs evaluating for possible intervention.
The problem is, if they intervene and re-establish a good supply to the left coronary artery, and the heart starts to improve in any way, there is still the issue with the valves being deformed. I have no idea, but perhaps someone else can say if mechanical valves tend to hold their shape more if the heart reduces in size again. If they surgically alter your existing valves, there is a good likelihood they will distort again if the heart shrinks back in size.

The main problem is that the pumping chamber of your heart, the left ventricle, is enlarged as as such is not beating efficiently which is why your EF% is low. It should be between 50% and 75% and it represents the amount of blood pumped out of the heart on each beat. The walls of your left ventricle are hypokinetic which means they do not contract properly, they are "lazy" which is why your EF% is low and you have Diastolic Dysfunction.

The cause would seem to be that you have severe regurgitation in two of your valves. This means that blood is being forced back into your heart due to the weakened valve which causes your left ventricle to work too hard which in turn causes it to become enlarged. Getting the valves repaired will take the pressure off your heart and help it return to a more efficient state.
