Generally speaking, mild valvular leakage as you describe is both common and normal. This may require a repeat echocardiogram in several years. The results of the holter monitor are also normal as your cardiologist has indicated. With exercise your heart rate should increase, and this is the normal bodily response. You maximal predicted heart rate is 220 minus your age ( or 183 or so for you). Also, 3 extra ventricular beats is also normal, especially in patients with mitral valve prolapse. The only thing I could suggest would be an exercise echo to evaluate if the mitral valve prolapse/regurgitation is playing any role in your exertional symptoms.
