Without being able to review your entire history it will not be possible to give you an exact answer, but the results of your echocardiogram are reassuring.  An echocardiogram is not so helpful at answering the etiology of chest pain however.

Based on the symptoms you have described, I might consider an evaluation for possible pericarditis or a gastrointestinal source as was mentioned earlier in this thread.  If the pain has an exertion all component then a stress test could be considered.  If you have a rapid heart rate then your doctor might consider a Holter monitor to make sure you don't have an arrhythmia.

Hope this helps!

Thank you for your thoughts.

I am sure the doctor will answer you soon, but as a patient who has had a lot of experience with of evaluations of my heart (I have chronic PVCs, exercise-induced bigeminy, and mild-to-moderate aortic insufficency), I will say that your numbers look excellent--extremely healthy. All your valves are doing their job well, and your heart's chambers are of normal size.

You do have a little leakage from your mitral and tricuspid valves, but that is tremendously common.  The small amount of noise heard from your minor leaks may be the murmur that you hear.  A BIG leakage that was giving you bad symptoms would be a different kettle of fish, but the description of your ticker looks just fine.

Your cardiologist thinks so, too:  " Impressi[o]n: Essentially normal "

Congratulations on that!

The heart is held pretty securely in a special space in your chest, so the likelihood is that the pain you feel, which eases when you lean forward, has some other source--maybe gut pain or something like that.

The rather fast heart rate you perceive (140-150bpm) is not terribly out of line.  It fits with panic attacks (which I also have).  Have you ever been checked out for signs of a problem with anxiety?  You might want to take a look at a general list of symptoms here:
