i have both pacemaker & defibrillator wired into my heart



what would my options be if one of th
e leads came loose & started to deteriate from defibrillator

If the implanted lead resistance/impedance is still within 300-1500ohms, its possible that you may undergo a lead reposition procedure but if not and its not possible to extract the lead, EP will use a lead cap and implant a new lead. Better have a pacemaker interrogation.

You could have isolated lead extraction if the generator didn't need to be changed.  A lead could then be reimplanted without having to change the generator.  

You should discuss with the electrophysiologist who implanted the device.
