What exactly is occult HBV or an HBsAG mutant?
What if the mom is hbeag positive?
no, but hbig is not so important and carry an extremely low and rare danger which is not zero danger
since it is blood extracted from people who has developped hbsab antibody it might carry hbsag mutants and occult hbv, infact a very low percentage of babies gets occult hbv but it is not yet known if from hbig
vaccine is the most important and mother with hbeag negative
Is there a way to test the child to see is they were vaccinated with HBIG?
yes he can be protected anyway.you must check hbsag, hbsab, hbcab and hbvdna pcr if all tests are free or cost is not a concern
if hbeag was negative and hbvdna undetactable/very low or very low possibily of infection is extremely low.about 5 to 10% dont respond to vaccine or get occult hbv when hbeag is positive or hbvdna high
there is possibility of infection and occult hbv infection but this is extremely rare, occult hbv infection can be found only with hi sensibility pcr hbvdna (6-12iu/ml sensibility) and it is better to recheck at 5 and 10 years old
the infection might happen thru placenta too and there is a small percentage that doesn t respond to vaccine or gets hbsag mutant or occult hbv
make sure abbott architect is used for hbsag and hbsab tests, it detects almost all hbsag mutants, if hbsag gets negative, hbsab positive with high titer, hbvdna undetactable the vaccine worked.just remake these tests at 5 and 10 years for maximum safety
