Precore mutant

Hi
I had posted before in July when u had a HBV flair. The ALT started from 298 and gradually dropped to 40 end of August. So did HBVDNA dropped to 800. Doctor ordered me another test which I did three days ago, however ALT is 80 and Viral load up to 30000. I am feeling sick and headache this week and having diahhrea. Will this cause the fluctuation?

According to my doctor, I am e negative carrier and is trick to treat. I also am a pre core mutant which means I converted to e negative when I was teenager. She said in my case fluctuation is common.

She said do a fibrous can to check if result no good ,I will be on interferon.

My ALT is always up the limit like 40 historically. Question: should I wait for the scan or start oral med now? I am thinking maybe my body is going through another clearance stage given it is fluctuating

No waiting is a waste of time since definitive hbv cure is very long and can be achieve in most patients by 10-17years, so the earlier the better

check the older posts about sequential treatment, peginf is good to cure hbv when hbsag baseline is less than 1000iu/ml and genotype is a or b, c and d are harder.check these before any therapy

if you have hi hbsag it is best to go for tenofovir, not entecavir, and after 3 to 5  years try the pegintf add on

Thanks STef. She said she can send it a lab in Melbourne. What is the significance of HBSgen quantity in my case in term of treatment? Do you think we should start treatment now? Or wait for the fibra scan and possibly the test result?
I am thinking since I had the flair in three months ago maybe I should wait a bit longer hoping things settling down?

Thanks!

Thanks STef. She can send it to a Melbourne lab for that. But what is the significance of this test in terms of treatment? Do you think I should start treatment moe or wait for fibra scan and the HBSeg quantity test?

U are in a country where hbsag quantitative is not performed (how this can be possible in a democracy is strange to me, private labs are free to do whatever in my country and we have this test since early 2000/2004).withoutthis test pegintf cannot be done and is blind on results

You should move or send samples to india, china, vietnam or others to see if pegintf is having a response.hbvdna or alt with no hbsag quant are useless
