Undetectable hbv dna and low hbsag

Hi All,

I’ve recently got my latest blood test its showing undetectable level of hbv dna and 10iu/ml of HbsAg , all other tests are normal , vit D is on the low side but within limit , the only other notable test was very high cholestrol which I have started tablets for.

Been a chronic carrier as far as I can remeber , tests has always been the same although this is my second time only to get hbsag quant , first one was almost a year ago and was 25iu/ml

I have read that interfron with zadaxin could be my best bet at this stage , any recommendation ?

I am not an expert. With your HBsAg < 10 iu/ml, according to Hong Kong research, you will have a very high chance to clear HBsAg naturally within the next few years. Personally, I would suggest taking a full course(3 doses) of the HBV vaccine (a recent clinical trial from Egypt reported a very high rate of HBsAg clearance). If you have access to Sci-B-Vac, it may be even better, According to Replicor, they concluded that with HBsAg < 1 iu/ml, Interferon will clear HBsAg and maintain HbsAg clearance.

Does replicor have this documented that less than1 iu/ml HBSAG.. interferon will clear virus?
You can see their late-breaker poster at AASLD 2017 from Replicor website.
They call it functional control in the poster... I don’t find a mention of functional control anywhere else. What you find is a mention of functional cure (which is ofcourse not in sight)

Doesn’t replicor poster reveal that it’s molecule could help gain functional contro (which they define as HBSAG less than 1 iu/ml, HBVDNA less than 1000 iu/ml)
According to Andrew Vaillant:
"REP 2139-mediated HBsAg reduction to below 1IU/mL dramatically potentiates the activity of pegIFN, that profound functional control of both HBV and HDV can be established in a high proportion of patients, that this function control persists for 1 year after therapy was withdrawn and is accompanied by normalization of liver function. The critical importance of HBsAg reduction below 1IU/mL was emphasized by the lack of potentiation of pegIFN in three patients who achieved HBsAg levels as low as 16.4, 5.74 and 1.88 IU/mL during exposure to pegIFN. "

With clearance of HBsAg, appearance of HbsAb, and durability over a year, I would call it a functional cure.

I have just done this test again , below are my update/corrected test results for HbsAg

July/16 : 122.9 iu/ml
Nov/17 : 7.0 iu/ml
April/18 :  4.3 ng/ml  ( not sure how much is this in iu/ml , one site says 7iu/ml ?

Hbv dna
July/16 : 27 iu/ml
March/17 :  < 20 iu/ml
Sept/17 : 21
April /18 : 47 iu/ml

ALT
July/16 :  33.4
Sept/17 : 30 iu/ml
April/18 :  56 iu/ml


AST
July/16 : 33.4
Sept/17 : 21
april/18 : 23 iu/ml

GGT:
July/16 : 28.43

April/18 :  85 iu/ml

Hbc : always positive
HbeAg : always negative
HbeAb : always positive


I have booked an appointment with a new hepatilogist in town and hopfully he will perscribe me interferon , what is the latest recommendation for this ? Combine with vaccines/zadaxine ?

Can you please also share a link to the honkkong paper and arrow head article , I’ll print and take with me.

i have a question..with your undetected hbv dna and low hbsag are you considered sexually infectious or not? what did your doc say? can you have sex freely?

No one can say he/she is not infectious untill HbsAg is cleared and Ab is formed , even then I would still make sure the other party has elevated level of immunity ( > 10 iu/ml of HbsAb) before unprotected sex is performed.

Yes, interferon treatment has a high chance to seroconvert you with this starting condition. But I doubt that you could get a prescription for it. Adding thymosin alpha additionally would further increase that chance for seroconversion.

Hi Happybbb,


My e antigen is negative , HbsAb is not detectable. I’ve never been on med.

I did read in this forum at some point that my result could be a good candidate to try out interferon for the objective of eradicating HbsAg

What about hbeag/hbeab?
Have you ever been on any hep b medication/treatment? If not then i dont think doctors would start treatment with your results.
