Which one is more effective-ETV+TDF or only TDF?

My mother has become resistant to both lamivudine 100 and entecavir 0.5, in that case which one can be considered a better option-etv+tdf or only tdf?Will there be any chance for resistance/side effects in the future?Uptil how many years can such combo be taken safely?

A clear case of entecavir resistance. After lam resistance entecavir should never have been given, even 1.0 mg does not work in the long run.

Tenofovir is the only choice and chance now.

Your doc is wrong. There is no reason to take entecavir and tenofovir at the same time. Both should just be taken at their respective time of the day.
Thus taking entecavir in the morning before breakfast and tenofovir with dinner will be just perfect.

After taking ETV+TDF for about 10 months my mom's HBV DNA has become negative.....etv+tdf is truly a very effective option remaining after being multi-drug resistant.....

Thank you for your reply......my mother takes ETV in empty stomach(2 hrs before breakfast).The doc. said TDF should be taken with ETV at the same time.Should I change the time at some other point of time of the day?as I am afraid of taking two antivirals in empty stomach early in the morning....what if it is taken at a time(2 hrs after breakfast and 2 hrs before lunch)?

He tests creatinine because tenofovir in some patients has a negative effect on kidney function.

Tenofovir disiproxil fumarate, brand name viread, can be taken with or without food. Slightly better absorption when taken with food can be expected.

Multi drug resistance now exists in your mom's hbv and the doc should not have used entecavir after lam resistance. It can be expected that the decrease in viral load under tenofovir will take longer, since the polymerase now has a mild partial resistance even to tenofovir, however thanks to the high dose the remaining power of suppression is still sufficient in the majority  of patients in this situation.

Today we visited our doctor who suggested my mother to take ETV 0.5 + TDF 300 once a day for 3 months and get tested for LFT,HBV DNA,CBC and Creatinine.Although my mom's hbv dna has increased to 250000 yet her sgpt.sgot levels have remained almost the same(40 and 41).He said it's a case of multi-drug resistance(lam+etv).Can anyone suggest when TDF should be taken before/after meals?Why did he ask to get tested for Creatinine after 3 months?Can there be any side effect?

no need for etv.. ur doc made very wrong decision starting lam. he should ve straight away started with tdf.. tdf has no resistance. keep monitoring u'll get good results on next check up.

Thank you for your reply.....After taking lam. for about 1.5 yrs my mother's hbv dna rose to 1800000 iu/ml.As a result of which she had to switch over to entecavir 0.5 and within 10 months the viral load fell down to 22000 iu/ml.Now after almost 1.5 yrs being on etv the viral load has again risen to 258000 iu/ml.Is this a case of resistance or etv 1.0 mg is needed?

tdf mono initially.. recheck viral load later if not considerably low then combo tdf plus etv
