good HBsAg tests in Paris?

I would be really thankful to know where in Paris I could go to one of these HBsAg tests:

Abbott Architect, Siemens Enzygnost HBsAg 6.0 or Roche quantitative Elecsys HBsAg II ?

What is exactly the name or model of Abbot Architect in order to avoid confusion?

Is there anyplace to test with NGI UltraQual in Europe?

Thank you in advance!

yes for occult hbv you do need NGI test, all others are not reliable.you have to send blood to them, try to contact them about how to ship the blood sample

the best way to check for occult hbv is also hbsag siemens test, the latest one, i posted it somewhere.it is the best on the mutants, i think the siemens can miss very few mutants maybe 1 or 2 while the architect can miss more
siemens is used where there is blood donors collection in italy so i guess it is the same in france.there are also pcr tests called NAT, i dont know how hey work but they have higher sensibility, i think these are performed on the same blood collection centers too

What should be the lower limit for anti HBcAb?

anyway hbcab igm less than 0.08s/co means inactive immune response but in your case i dont know how reliable this can be, hbsag mutants are correlated with changes in hbcag and dna polimerase, i dont know if always or sometimes just changes in hbsag

Thank you, that is clearly said. I will start searching. The fatty liver option has been excluded.

The siemens assay must be Enzygnost HBsAg 6.0.

I'll keep informed once I have more results.

What should be the lower limit for anti HBcAb?

not important, i think a siemens hbsag test+ hbvdna nat or ngi will sort out any doubt

are you sure the sympthoms can be from hbv.did you check fatty liver, nash?

Thank you Stef very much for your answer! I am still a little bit confused. My case: I had a blood contact with a dialysis patient. All my tests where negative 6 years ago. Recently I was told that mutations of HBsAg are common with dialysis patients. I have symptoms and now searching for good tests with mutation detection and in case of occult hepatitis B a good DNA test.

I refer to an article about Occult hepatitis B virus. I quote:

"...the median HBV DNA level in a case of OHB is 32– 62 copies/mL or 5–10 IU/mL (range <10–425 copies/mL) ..."

and

"The preferred lower limit of detection (LLOD) for HBV DNA is <=5 IU/mL or -30 copies/mL." (F. B. Hollinger, G. Sood, Occult hepatitis B virus infection: a covert operation, 2009)

Should I still search for NGI UltraQual for low levels of DNA? Is abbott architect enough for mutation detection or is there other tests that should be considered? What should be the lower limit for anti HBcAb?

Thank you very much again for giving some light on this issue!!

just ask for abbott architect hbsag quant which is tthe easier to find.the main problem with the labs is lazy tech dont make diluition to detect the values higher than 250iu/ml and we all have higher values....
so double check if they make diluition to give you the exact number in iu/ml, the range is usually hbeag neg 3000- hbeag pos 20000iu/ml on cronic carriers

to test with NGI UltraQual in Europe?
no, only in LA but this is not useful for hbv because the replication cycle or clearance is not correlated with hbvdna at all but to hbsag which is correlated to cccdna
i dont see any benefit to make NGI ultraqual for hbv, the only benefit can be checking for a very low hbvdna in fear of resistance from use of entecavir monotherapy but not very reliable because hbvdna flactuates and can be 10iu/ml one day, 1iu/ml the other and so on...

an hbsag value lower than 1500iu/ml, especially if as low as 500iu/ml, can tell you you are reaching immune control and low cccdna values.this is much more reliable than hbvdna ultraqual
