I am hbsag positive hbeag negative anti hbs positve,; alt 11 ast 18
hbv quantitative befor viread treatment it was 8505 iu/ml
after 6 months with viread it became 58650.3iu/ml (abbott m200)
hbv DNA it inwas 1100 know undetected.. what do you think of it? Should I continue with treatment? I would appreciate it if you would explain to me a bit.Thank very much.

not 6 months, 3 to 5 years on 50% of treated patients only.in case of no hbsag decrease at all peginterferon can be tried and see if hbsag goes down by 12 weeks of combo
only those with hbsag less than 1000iu/ml have high chances to clear on pegintf add on to tdf after 3 to 5 years of tdf mono
so it is a slow process.you can check your hbsag quantity in iu/ml, only architect and elecsys to check quant in iu/ml so check tha lab has this machine for quant before doing the test.hbv genotype is helpful too
after we have these tests we know if tdf or peg is the best choice

it is an old view that making hbeag negative is good, when it happens out of therapy it is always a mutation of the hbv virus to precore or bcp for genotype D and a worsened state of infection with time.other genotypes might have lower rates of mutation
the only good thing is:
tenofovir for 3 years or more (in case of hbeag positive hbv is cleared in 16% cases already)
if hbsag is not cleared by 3 years of tenofovir (hbsag cleared means hbv cleared):
add on peginterferon to tenofovir for another 2 years
in the end most clear hbsag (hbv) and develop hbsab antibodies, few patients reach a very low hbsag quantity which determine an inactive state of infection which is slowly cleared by the years of by tenofovir monotherapy
total time to cure hbv by sequential treatment 4-5 years, 50% clears in 4 years the rest of patients have very low hbsag levels and might clear in the following years
