I guess that your wife has liver disease or she wouldn't be taking it, right? A lot of people would probably caution against it while pregant, so I would consult her doctor and/or a liver specialist. I took it for about six months before starting tx, but didn't notice any difference, although I didn't have much in the way of symptoms and actually, my VL doubled to over 1,000,000 during that time. UPenn has been granted a trial from the government to study Milk Thistle, but that won't help make your decision during the pregnancy. Best of luck with the pregnancy.

I don't know if any long term studies have been done on milk thistle and pregnancy. My feeling is that she should hold off on herbal therapies until after delivery.
I took Maximum Milk Thistle for long time before I started treatement with transactions, Maximum Milk Thistle is excellent in my experience, your wife may feel beter with that and it also may slow down the progresion of the desease.
But don't take milk thistle while on Tx because it may reduce the impact of the medicine.
Since she is pregnant I would also ask a liver doctor, just to be more careful.
