lost response on 10th months of tretmentC

Hi Guys,
Please help, I am totaly stress out. I have been on hepC treatment for 10 months and I got undetectable
level month 4. Now month 10 it become detectable with the level 100 and after 3 weeks. even worth 280.

My white blood count is droped too recently. Do I have any hope stop and continuen after a while.
My immune system down, totaly I've been on treatment 16 month, 6 on pegi-interferon, 10 months
on infergen (dayly interferon). During all this time I went through a lot... And now it so sad-just 2 month befor finish treatment..????

your change of labs may have been the cause of inconsistant results.  my dr said if i had viral breakthru,  i still would need treatment for 6 months,  for 1b.  i was UND during treatment @ 3 months.  will have been off meds for 1 month & due labs monday.  try to go to the same lab & request copies of results,  for your records.  my dr uses the quantatative RNA test from quest lab.  good luck--- you still helped your liver to heal even if you did breakthru.  kudos132

Next-Generation HCV Protease Inhibitor SCH 900518

As part of its long-term commitment to hepatitis C therapy, Schering-Plough also is developing SCH 900518 ("518"), a next-generation HCV protease inhibitor. A Phase IIa study with 518, known as the NEXT-1 study, is currently ongoing. The company said that 518 has been shown to be 10 times more potent in-vitro than other protease inhibitors currently in Phase III development and has the potential for once daily dosing. 518 also has shown decreased emergence of resistance in vitro. Given its pharmacokinetic (PK) profile, the company anticipates that 518 may be active against some HCV strains that are resistant to other protease inhibitors. Phase I proof of concept studies with 518 in treatment-naive patients and those who failed prior treatment, both as monotherapy and in combination with peginterferon (without ribavirin), demonstrated enhanced antiviral activity, with up to 4 log10 and 5 log10 decreases in circulating HCV, respectively.

Full results of the boceprevir HCV SPRINT-1 study and early phase clinical results with SCH 900518 are being submitted for presentation at a future medical meeting.

Hi there,

Am I reading your posts correctly? You were UNdetectable at 4 months, right? When did you do the super dosing...before or after becoming UND? And then you returned to your normal infergen dose?

  I ask because I too was higher dosing early on then was reduced to SOC for a couple weeks due to Psych issues. I felt the worse EVER on the usual dose.... more sx and URQ pain. I had fought my Dr. about re-upping my doses again, but then I began to worry that I was relapsing so embraced the idea after all...and oddly feel lots better.

About the new drugs, they are only available in clinical trials right now. As some of the others have said some may hit the market in 2011 for those who don't have the option of a study or clinical trial. I had a "blip" like you did between my 2 week and 4 week PCR. It did come back down at week 8 but still not UND. My next one is next week and will be the determiner of what I do. I may be given the option to add the protease inhibitor if I'm not UND but have showed some results. I'm not sure off the top of my head what the criteria for "some results" is, but if given the option I will almost certainly add the drug. The PIs show a lot of promise, and the possibility of 24 week TX time for geno 1s. I won't get that option since I would be doing a cross-over arm, but to me, that is an exciting aspect of these new drugs. I am very sorry to hear you got this news, especially after so much of your time was invested. Best to you.

Hi Bill,
Thank you for valuable and a little hopfull comment.
I did convince my doctor to extend this tx for 1 month and do research now.
I am not sure about PCR or TMA, but test was done in different lab, we just moved
from WA State to California.
Another issue - I was overdosing myself, couple days befor regular tests for 3-4 months.
But last 2 months I did not do it and it become detectable, I thought it coinsedence.
Now in CA State I found hepatologist and will see him April 21, I'll let you know

Sometimes things get lost here in translation and/or incomplete information -- but that said, I don't understand why they continued treatment after you were detectable at week 16 and more so why they still continue treatment. Have you been given an adequate explanation from your doctor? Is your doctor a liver specialist (hepatologist)? What stage liver damage do you have? Unless there's more here, personally I'd get another opinion from a liver specialist (hepatologist) at this point.

I’d be very careful discontinuing treatment right now; although it does appear you might have suffered viral breakthrough, this might possibly be a ‘blip’ in testing. Do you know what methodology is being used to detect virus? It looks like the doctor is ordering quantitative tests; are they PCR? TMA? You might want to ask for two separate tests; one possibly a sensitive TMA qualitative, along with a bDNA test to provide another view. You have considerable time invested in this treatment; tread carefully.

The first new drug is called telaprevir-it will be out in 2011.It is not uncertain,it is virtually certain to be the next generation of treatnment and will be taken in addition to interferon and ribavirin.
There is a revolution in standard of  care for hepatitis C just around the corner.

The new drugs are coming, perhaps 2011, perhaps somewhere between 2012-2014. They are looking very promising. It is protease and polymerase inhibitors added to the pegylated interferon and ribavirin of today. They are at the present only available in clinical trials.

You might want to read about study 107, which includes people with viral breakthrough and look at the graphs which show how different patient categories respond.

Sorry to hear of your situation but you have done what your name says and now you need to learn to live with your HepC until you can tx again.  Give yourself a good long rest.  good luck

So sorry, but if you have become detectable again during treatment (viral breakthrough) you have no hope from this round of treatment.
The virus has beaten the interfron and further treatment will only cause you more distress.
You will have to treat again using a new modern drug added.
