HSV Blood Test Results Question

Hi,

I recently got a follow-up herpes blood test as of mid March (13 weeks after last exposure), and I don't quite understand my online results. Unfortunately, I have not had any communication yet from my dermatologist. The results are as follows:

HSV I/II I igG Elisa QL 1.62
HSV2 glycoprotein G Ab IgG 2.75
HSV1 glycoprotein G Ab IgG 0.15
HSV I/II IgM Elisa QL 1.29

From a historical perspective, I was initially tested in late Dec and received my results verbally from my primary care physician's office... The results were negative for IgG but higher for IgM which suggested a follow-up test.

From a background standpoint, I was last sexually active within a relationship in very early December and June of 2013 prior to that. Nonetheless three days after that last contact in Dec, I had discomfort with urination. My now ex-girlfriend has had no issues but is very evasive on the subject leading me to think she is potentially asymptomatic or in denial.

Going forward from Dec, I did have bumps develop that resembled insect bites on my left buttock in early January. My dermatologist thought it may have been zoster since only one side of my body and bumps formed in a straight line. They did a culture of the bumps early, but it did not register either way. The bumps bruised and then healed without any consistent pain other than an occasional ache below the skin at times.

As of yesterday, I developed a cold sore on my upper lip, my first ever... I have not been active in any way since early Dec. So, I am a little confused. I do understand that with this information, it is difficult to provide anything absolute. However, I would appreciate any feedback on my latest test results, suggested next steps or conclusions in your opinion.

One last question, I must admit that I have consistently gravitated to the 2.75 lab result since the larger IgG numeric. Is the other IgG value (1.62) under Elisa QL less important? Lastly, what does Elisa QL mean?

you have a low positive hsv2 igg. you will need to follow up on this with further testing. if you are in the US, a herpes WB blood test is recommended 4 months after you last had sex.  

should you get a return of symptoms, be seen within 48 hours of their appearance for a lesion culture and typing.

unfortunately even the best blood tests still miss 1 out of every 10 hsv1 infections. if you haven't kissed anyone since december, it obviously isn't a new infection, just your first obvious recurrence. No easy way to determine if you contracted hsv1 orally from your last partner or had it previously and just didn't know it.

Herpeselect is a manufacturers kit. You could have had this test but the way the results are presented seems otherwise. I don't really know.

A level like 2.75 could indicate a rising level of antibodies more so than a longer term infection.

Thank you for your quick feedback, I am surprised that my second set of test results (March 2014) would indicate a past infection since a prior negative for IgG in Dec 2013. What indicators from this March test raise that suspicion?

Secondly, you mention this not being Herpeselect if I understood you correctly... What type is it for reference?

I hope to hear from my doctor soon and may proceed with a Westernblot depending on her advice.

I'm not sure about the cold sore on the lip, as you've never had one before I suppose you can't really be sure if it is.

The results indicate that there is a suspicion that you have been infected with HSV2 in the past. The value looks at a reasonable level for a positive but not being Herpeselect there are no chances we can ascribe to this.

If your medical advice is still that this is indeterminate, then consider a Westernblot test for HSV and the answer you can take as definitive.
