Is a mild first outbreak?

I am a 36 year old, otherwise healthy, married Caucasian male.
3.5 weeks ago, I was the recipient of unprotected oral sex from a prostitute, followed by protected vaginal sex (condom). 5 days later I found a red, slightly itchy, patch on my testicles that looked like poison ivy. On day 7 following my exposure, my primary care Dr. said it wasn't herpes and prescribed antifungal ointment with a steroid. I got urine and blood tests done which came back negative.

A few days after my Dr appt. I noticed a pimply rash on the top of my left foot. Followed by a rash on my anus and my testicles had become red all over. On day 15, I went to Planned Parenthood with these new symptoms. Dr there said it wasn't an std and added Desitin diaper rash cream to my treatment.

A few days later, my red testicles developed a few small pimply (white, not itchy) outbreaks and the shaft of my penis got deep red. I focused on drying out my groin and bought Lamasil Ultra since my prescription ran out. The pimply outbreaks on my penis scabbed and went away, except for one that is still there. I masturbated once and my red penis got worse, especially under the head. The anus and foot rash is gone.

It's been 25 days since my exposure. My testicles are still red with one small pimply dark red spot. Under the head of my penis is deep red and tingly. The shaft of my penis has dry red crusty rings which look like scabs from drying my penis too much (but I'm still worried). All these symptoms have been tingly, not particularly itchy or painful. They fade if I wrap my penis in toilet paper and leave it like that to keep the area dry.

I am particularly worried about trusting the doctors and exposing my wife and 2 year old child to an incurable std.

My next step is a 4 month blood retest. Should I or could I do more? What does this sound like to you?

I guess you will just have to wait and see what happens with the scratch.  If it gets greater in size, you could go in and have it swab tested.

This morning I found a small scratch/sore on outside of nostril where it meets my face. If it's a cold sore, will it grow? Or how can I discern it from some random scratch or pimple. I am feeling so down that I can't go more than 24 hours without another symptom worry.

I doubt that something as spread from your testicles to your mouth.
These symptoms sound like they could be fungal but very hard to say, honestly.  
Just for the record, I'm not clear that you were tested for herpes 10 years ago as part of an STD screen.  You could see if you could get a copy of your records.  
At this point, I'm not sure what you can do except wait and get an antibody test.  If you get any more bumps, I would go once again to a clinician and see what they think.

Update: I developed what seems like oral thrush symptoms last night. I thought they were canker sore again and worried it was oral herpes again but after online research this morning, I just brushed away the white cottage cheese like layer. There wasn't too much, but the inside of my lips feels a little numb now.
Could this have spread from my testicles?

Current scaley dry skin circling shaft follows creases of excess skin on flaccid penis. Scabbiness on testicles is lots of little spots, almost website. Could befollowing creases of loose sack skin, but less sure.
I'm at day 25 and feel most "clean" since day 5.
Today is the first day in a week I've chilled out on drying out my groin with toilet paper and max strength desitin.
However the scabs and single small patch, one eighth of an inch in diameter have me most worried.
I've freaked out about two canker sores on inside of lip in the last week, but they both disappeared in less than 24 hours.

The rash is/was red never really itched outside what made me notice the first clump. A few small spots turned white like a whiteheads for 24 hours. I actually booked a third apt. at that time for fear they were herpes sores. I wanted a Dr. to see them and possibly take a culture. In 24 hours they were gone so I canceled appt. However fear returned because tiny scabs appeared there that night.
One tiny red cluster still remains on my testicle today.
I've never had a cold sore anywhere. I've been fully std tested before being monogomous with wife for past 10 years. All clean.
I haven't used condoms for those 10 years apart from this experience. Before and this time I have had no trouble. This was my only transgression.
I was tested and cleared for studs incl. Herpes at day 7 of this process and given clean health.
I have not had sex with wife since encounter with prostitute.

As always, a clinician looking at a something on someone's genital area is far better than me guessing over the internet.  
I'm sure what to make of what you describe as a white pimply rash on your testicles.  I'm also concerned when you use the word "scabbed".  
Have you ever in your life had a cold sore on your lips or in your nose?  Do you normally use latex condoms without difficulty?  
Have you been tested for all STDs somewhere along in this process, including for herpes?
Have you had sex with your wife yet?
If you answer these questions for me, it will help me respond accurately.
