What does this mean?

Hi! I'm hoping someone can help me sort out this mess. Here is the background. I started having headaches and fevers on Wed the 10th, they continued on Thurs the 11th. My glands in my pelvic area began to swell on Friday and by Saterday I had 2 large scist-like ulcers on my labia. Sunday more appeared, it was painful to urinate and I began getting sharp pains in my groin. I had been hoping I could hold out until Monday and go to my doctor but I found myself in the ER Sunday night. That was Sunday the 14th.

They were in the middle of shift change so 2 different ER doctors looked at me. The first said that the symptoms sounded like herpes but when he looked at me he said that the area did not look like a herpes infection. The (lesions?) were too big he said and not grouped together. Then the second doctor looked and told me that he was sure it was herpes, but that the first outbreak is usually much more severe than later ones.

They took a culture and presrcibed me acyclovir 800 mg 5 times a day for 10 days.

The next morning I went to see my general physician. She didn't give me an exam. She looked at my discharge instructions, my meds and listened to me. She said that she was not going to assume anything and that she wanted the results of the test first but to go ahead and take the meds. She also told me to call if i hadn't heard from them in a week.

Today marks a week from that visit. I am much much better. It no longer hurts to urinate and most of the lesions are gone. I am a bit itchy and my anus is very sensitive and i bleed a bit with bowel movements. I called the dr office this morning and was told I would have to make a appt to get the results of my test. But the hospital called me today to check on me and i asked them for them.

The nurse at the hospital told me that my results are "Type 1 Isolated" but that she didn't understand that and I would have to talk to my doctor.

I have made an appointment. But I have also been looking online and it seems that is a positive? What is the difference between Type 1 and Type 2? and what does Isolated mean?

1 out of every 2-3 adults has hsv1. It's incredibly common. It has little to do with sex - about 1/3 of us contract it even before we step foot into kindergarten. It's transmitted most of the time through kissing but if you didn't manage to pick it up orally before you become sexually active, you are vulnerable to contracting it genitally when someone who has it orally performs oral sex on you. This is most likely what has happened for you. You can transmit it from genitals to genitals but it's most commonly transmitted through oral sex. There doesn't have to be an active cold sore present in the infected partner in order for the virus to be active and transmitted to a partner.

having hsv1 genitally isn't much of an issue as far as having a family. Recurrences are few and far between for most people so it shouldn't be much of a disruption for the getting pregnant part. Recurrences during pregnancy are a comfort issue only and not a risk to your unborn baby since you've had it prior to becoming pregnant. Should you have a recurrence at the time of delivery or close to it, a c section is usually performed to protect the baby. It's a less than 1% risk of transmission to a newborn during delivery in the absence of symptoms at the time of delivery. when you do  become pregnant, we can help you much more than :)

I was tested for everything in October of last year after finding out that my husband was unfaithful several times. I came up negative for everything. My partner hasn't been tested in a very long time. He just came out of a long term relationship. We do not use condoms because I am on the depo provera shot and we are both serial monogamists. Ignorantly we assumed that because he just had a baby a year ago and i was just tested a year ago with only one other safe sex parter before him since the testing that we were safe. Guess thats what we get for assuming anything. We do have oral sex. In a way I am glad I caught it from him. I would rather have got it from someone who I know loves me and did not know than from someone who is cheating and careless. I have known him for 8 years and he is my very best friend. Does this mean that his previous spouse cheated? He has never had an outbreak. He plans on getting tested once we are done with my diagnosis. What does this mean in regards to having children?

yes this sounds like a newly acquired infection so it's likely it did come from him. do you two have oral sex? ( no secrets here...lol )

not too unusual to have bleeding issues with a primary infection.  this is a lot of stress on your body.

have you two had full std testing since you started having sex?  do you use condoms?

Thank you vedad and grace. :) I'm very happy I found this site. I understand the difference between the types now. I have a regular partner and we have been monogamous for about 4 months. He is very very supportive. I don't understand the incubation period with this. The doctor said it is 2-21 days? So this had to have been from him? It's all so confusing. :( I do believe this is a hemerrhiod. Something else, when I first started having symptoms I was on my 2nd day of my period. It stopped that day, and now i am on it again. Is that normal? I usually bleed for at least four days and have 28 days between periods. I should not start again for at least 2 weeks...

If you had a lesion culture come back as hsv1+, that means you have hsv1 genitally. it's pretty common , especially in folks under the age of 30.  

So what do you do now?

odds are you contracted this from receiving oral sex from your partner. Do you have a regular partner? This sounds like a primary ob of herpes so odds are it's from whatever partner you had last.

hsv1 genitally tends not to reoccur very often and doesn't shed much either. If your partner already has it orally, it's really a non-issue other than avoid sex when you have obvious symptoms.  the free herpes handbook at www.westoverheights.com has terrific info on genital herpes for more reading. I also encourage you to watch Terri's patient counseling video on there too.

I encourage you since you are still itchy, to return to your regular provider and get properly tested for yeast and bacterial infections vaginally too. Bv especially is very common with the first ob and can prolong your discomfort as well as cause herpes to reoccur. Make sure they do proper testing, not just a looksee.   Since things are healing up, this will be less sore for you to do now.

since you are bleeding with bowel movements, I encourage you to keep things as soft as possible coming out of that end. Take a stool softener like colace. have you ever had hemorrhoids before?  You can also buy tucks pads ( generic is fine ) and use them as a compress in the anal area. Hold them in between your butt cheeks and practice your duck walk to hold them in place for 20 minutes several times a day. these will help with swelling, pain and itching no matter what the cause of your bleeding.

But as the gentical make up of virus is different from bacteria, so advanced techniques like PCR are done to confirm the presence of virus.

The identification and isolation for every pathogen is different but it is done by a Lab Technician (Microbiologist) for study and research purposes.In a hospital set up it suggests the Doctors to try different drugs and decide to finalize the treatment options.
